In Home Clinician– PACT-Y

Wayside Youth & Family Support Network’s Milford site is hiring clinicians for an innovative, evidenced based model of treatment in Massachusetts, the Program for Assertive Community Treatment for Youth (PACT-Y). This program is a DMH funded, intensive, self-contained, community-based program, serving youth and their families. We are looking for individuals who are open to working a flexible schedule. We have 3 clinical position schedules needed: • M-F with evening hours • Su-W with evening hours • W-Sa with evening hours * Schedules have the potential to be adapted to include 10-hour days over 4 days as long as each day of the week is covered across the three schedules. What you will do:
- Work as part of a strong, collaborative, multidisciplinary team to provide mental health services to a small caseload of youth and young adults up to age 22. Ensure close collaboration and coordination of care with youth, caregivers and providers.
- Deliver services where and when they are needed (mileage reimbursed).
- Conduct individual and family counseling using behavioral, trauma informed and culturally responsive interventions for youth and their families.
- Encourage youth to have an active voice in their treatment.
- Assist caregivers in navigating systems, to promote their awareness of resources and access to supports.
- Empower caregivers with skills and strategies to support their child’s behavioral health needs.
- Create care plans and interventions to support skill-based growth for clients.
- Promote and strengthen family’s natural supports to foster a welcoming and supportive community for the youth.
- Ensure that client’s rights are respected, particularly in regard to cultural competency.
- Collaborate with other community-based organizations to provide services and care for clients.
- Provide on-call support as part of a rotating schedule.

Minimum Requirements:
- Masters Degree in Social Work, Mental Health Counseling or related field.
- Driving in the community is a requirement of this position; please ensure you have a valid driver's license and access to a reliable vehicle.
We will train you in: Motivational Interviewing; Courageous Conversations; Trauma Informed Practice; Attachment, Regulation, Competency Model (ARC); Substance Use Intervention; Risk and Safety Assessment.
