In Home Clinician– PACT-Y

Worcester County/Milford

 

 Salary Range: $65,000.00 - $70,000.00/year depending on experience + sign-on bonus (half paid after 30 days of employment and half paid after 90 days of employment; pro-rated for part-time).   

Wayside Youth & Family Support Network’s Milford site is hiring clinicians for an innovative, evidenced based model of treatment in Massachusetts, the Program for Assertive Community Treatment for Youth (PACT-Y). This program is a DMH funded, intensive, self-contained, community-based program, serving youth and their families. We are looking for individuals who are open to working a flexible schedule. We have 3 clinical position schedules needed: • M-F with evening hours • Su-W with evening hours • W-Sa with evening hours * Schedules have the potential to be adapted to include 10-hour days over 4 days as long as each day of the week is covered across the three schedules. We provide a supportive culture that allows for a balance of personal responsibilities and professional development while contributing to the community.

What you will do: 

  • Work as part of a strong, collaborative, multidisciplinary team to provide mental health services to a small caseload of youth and young adults up to age 22. Ensure close collaboration and coordination of care with youth, caregivers and providers.
  • Deliver services where and when they are needed (mileage reimbursed).
  • Conduct individual and family counseling using behavioral, trauma informed and culturally responsive interventions for youth and their families.
  • Encourage youth to have an active voice in their treatment.
  • Assist caregivers in navigating systems, to promote their awareness of resources and access to supports.
  • Empower caregivers with skills and strategies to support their child’s behavioral health needs.
  • Create care plans and interventions to support skill-based growth for clients.
  • Promote and strengthen family’s natural supports to foster a welcoming and supportive community for the youth.
  • Ensure that client’s rights are respected, particularly in regard to cultural competency.
  • Collaborate with other community-based organizations to provide services and care for clients.
  • Provide on-call support as part of a rotating schedule.

Minimum Requirements:   

  • Masters Degree in Social Work, Mental Health Counseling or related field. 
  • Driving in the community is a requirement of this position; please ensure you have a valid driver's license and access to a reliable vehicle.
