PACE Transportation Coordinator
Position Summary:
Under the direction of the IDT Manager, the PACE Transportation Coordinator is responsible for coordinating, scheduling, and arranging necessary transportation services for all PACE participants and potential enrollees. Participates as a member of the Interdisciplinary team. Responsible for the creation of the daily companionship manifest.
Essential Functions of the Job:
- Participate as a member of the Interdisciplinary Team (IDT).
- Attend meetings and take part in the approval of services, care planning and communicate changes in participant condition to IDT team members.
- In conjunction with the contracted transportation provider, plans and coordinates all transportation scheduling activities related to San Diego PACE participants.
- Coordinates with other IDT disciplines, participants transportation needs that may involve, companionship and translation services.
- Responsible for the coordination of transportation services, and for entering necessary information into the Participants medical record according to PACE regulations.
- Maintain close communication with participants and their families/caregivers.
- Check-in call to participants for initials and reassessments.
- Properly documents in participant’s medical record all communication, and relevant information regarding participants’ transportation needs and interactions.
- Answers telephones and transportation inquiries in a timely manner.
- Utilize transportation software to provide timely information to participants and PACE staff regarding estimated time of arrival and/or transportation status.
- Create and provide companionship manifest to IDT manager daily.
- Perform reminder calls when needed to participants to confirm specialty appointments including the need for companionship 48 hours prior to appointment date and record outcome on Participant’s health record.
- Removal of participants that are disenrolled or expired on transportation software.
- Coordinates, monitors, and maintains accurate records and data of day-to-day transportation activities to provide information to IDT Manager as necessary.
- Address service problems that may arise and communicate concerns to IDT Manager and Technical Manager as necessary.
- Transport participants in PACE vehicle when needed.
- Visits to participants home may be required.
- Performs other related duties as assigned or requested.
Additional Duties and Responsibilities:
- Maintains and complies with established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.

Job Requirements
Education Required:
- High School Diploma/GED
Certifications/Licenses Required:
- Current CA driver’s license and access to an insured vehicle.
Experience and Required Skills:
- A minimum of 6 months’ experience in a healthcare-related field.
- Experience with transportation coordination and/or working with the elderly would be very helpful for this position.
- Demonstrated ability to handle multiple ongoing department requests, ability to work under minimal supervision and ability to meet deadlines.
Verbal and Written Skills Required to perform the Job:
- Bilingual Spanish and English.
- Excellent organizational and communication skills.
- Good public relations.
Equipment Used:
Computer, telephone, fax, copier, and other office equipment. Must be able to use computer proficiency.
Working Conditions and Physical Requirements:
Physical activities include prolonged periods of sitting while working at computer. Frequent walking and standing. Driving of personal and/or company vehicle. May be able to work weekdays, weekends, and holidays per operational needs.
Universal Requirements:
