DHHS System Navigator

Department of Health and Human Services

Hourly Rate:  $26.45 - $30.49

Job Summary:  The DHHS System Navigator provides reception support to participants, including walk ins for Housing Services. This position is responsible for managing clients for all Housing Services programs, answering relevant questions, and providing the public with as many community resources as possible. To further our No Wrong Door philosophy, this position will be a first point of contact for our participants and assist with navigating through the system to ensure participants connect with the appropriate contacts and potential services. The DHHS System Navigator will collaborate with other service areas to ensure appropriate referral processing and sharing of information between service areas.

 

Job Duties:

  • Provide excellent customer care for all walk ins at the front desk through a solution focused approach. This will include assisting participants and landlords with questions regarding rent assistance payments, homeless walk ins and connection to resources, and any other walk in looking for housing resources.
  • Provide immediate assistance and education when applicable as well as coordinate with other system partners to ensure that the participant is being connected to the appropriate contacts and getting access to the correct services. Serve as a liaison between participants and program staff.
  • Assist with maintaining community resources up to date and available for all Housing Services participants. Assist with managing the resources share tool for program staff and participants/landlords.
  • Support all Housing Services programs with data management projects, as needed.
  • Perform other duties as assigned.

 

Minimum Requirements:

  • Current resident of the State of Wisconsin.
  • Possession of a high school diploma or G.E.D. equivalent
  • At least one (1) year of work experience in a customer service role, case management, and/or experience working with individuals with mental illness.
  • At least intermediate know of all computer and software applications such as Microsoft Word and Microsoft Excel.

 

Knowledge, Skills, and Abilities: Knowledge of housing programs and HUD regulations as they relate to homelessness and income guidelines; knowledge of available housing resources; knowledge of the permanent supportive housing model; ability to work with individuals with a wide range of needs.
