Clinic Licensed Practical Nurse, Part-Time, Day Shift
Community Care is hiring a Clinic Licensed Practical Nurse to join our team. This is a part-time position 32 hours/week with no holidays or weekends at our Waukesha clinic.
Under the supervision of the Program Manager, provides nursing care and other services supportive to the provision of care for members. including frail or elderly population and/or a developmentally or physical disabled population with diverse ethnic and cultural backgrounds. As a member of the interdisciplinary team the Clinic Licensed Practical Nurse works with members to increase independence and community integration through a model of self-determination which maximizes physical, mental, and functional health.
Clinic LPN Job Responsibilities:
- Triage - Performs on-site and phone triage using evidence-based triage protocols to identify and intervene for existing or potential member health
problems (physiological and psychological). - Scope of Practice - Performs technical skills within the LPN Scope of Practice.
- Education - Provides member, family and caregiver education.
- Residential Visits - Collaborates with care team to facilitate urgent residential visits when needed.
- Member Treatment - Confers with Primary Care and other providers for further evaluations and treatment needs of members.
- Interventions - Evaluates the effectiveness of urgent care interventions.
- Member Care Monitoring - Delegates appropriate member care to support staff within the clinic. Collaborates with care teams to plan, develop and implement member care.
- Process Improvement - Contributes to the improvement of triage, urgent care and clinical nursing practice by working with leadership to refine
and develop improvements in process flow and member care. - Nursing Protocols - Participates in development of nursing triage and nursing protocols as well as other committees.
- Additional Duties: Assists with daily clinic needs. Assists members in day center as needed. Schedules appts for members as needed.
Clinic LPN Job Requirements:
- Experience: Required - Minimum of 1 year recent exolder adult or persons with developmental and /or physical disabilities in a hospital, home casre or nursing home setting. Preferred - 1 year of practical nursing experience.
Clinic and phlebotomy experience - Education: Required - Licensed Practical Nurse/LPN Diploma from accredited program in practical nursing.
- Certification: Current CPR/AED certification
- License: A current Wisconsin Licensed Practical Nurse/LPN license in good standing
WI Driver License
Clinic LPN Job Schedule:
- 32 hours per week
- 8:00AM - 4:30PM
- Monday - Friday (1 weekday off - no weekends/holidays)
