POSITION SUMMARY

Thrive Skilled Pediatric Care’s purpose is simple, to provide high quality clinical home care to medically fragile children so they can grow and flourish to their full potential. The Licensed Health Aide (LHA) provides nursing assistant services including activities of daily living and personal care services, bowel and bladder care, medication administration, enteral care, therapy and tracheotomy care in the home in compliance with a plan of care for their child.

HIRING REQUIREMENTS

  • At least 18 years of age.
  • Possess a high school diploma or GED.
  • Be able to communicate and understand and return communication effectively in exchanges with others involved in patient care.
  • Is a parent, guardian or legal family member of an individual who is under 21 years old and who is eligible to receive skilled nursing services.
  • Successful completion of an approved LHA Training Program
  • Successful demonstration of a supervisor-observed competency skills and exam.
  • Holds a current Health Aide License in good standing with the State of Arizona Board of Nursing.

 

      ESSENTIAL RESPONSIBILITIES

  • Assist patient with activities of daily living including bathing, skin care, hair care, nail care, mouth care, shaving, dressing, feeding, ambulation, exercise, transfers, positioning
  • Provide monitoring of patient’s vital signs and blood pressure.
  • Provide bladder and bowel care, enteral care and therapy, tracheotomy care and medication assistance as per patient’s plan of care.
  • Monitoring and documenting of any patient changes, as well as reporting results to the supervising RN and/physician.
  • Observes and maintains the home environment to ensure the safety and security of the patient. Ensure patient living conditions are as comfortable and safe as possible by keeping patient rooms clean and tidy and making beds, adjusting room temperature and lighting, removing potential safety hazards or sources of personal injury.
  • Timely completion of required care/service notes documenting services provided during each shift.
  • Practices standard infection control precautions and basic first aid.
  • Completes appropriate educational trainings as applicable.
  • Attends and participates in appropriate care coordination conferences, staff meetings, and Thrive SPC committee meetings.
  • Performs other related functions as directed by supervisor.
  • Maintains confidentiality in all aspects of job performance.
  • Complies with all applicable laws, regulations and standards in performance of job functions.

  • Ability to complete required initial and recurring annual trainings/certifications

  • Maturity and ability to deal effectively with the demands of the job
  • Ability to read and write in English
  • Must understand and respect client including ethics and confidentiality of care
  • Ability to follow safety rules, adhering to legal and ethical standards of care
  • Ability to identify situations where skilled home health care/a licensed professional would be needed to meet higher needs of the patient, in the following tasks: skin care, ambulation, bathing, dressing, exercise, hair care, mouth care, positioning, shaving, bladder care, bowel care, transfers, medication administration, respiratory care,  enteral care and therapy.

      TRAVEL REQUIREMENTS

  • Travel to patient home and office locations, as necessary.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. 

This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting patients.
