Home Health Aide
POSITION SUMMARY

The role of a Home Health aide is to administer care to patients under the ongoing direct and/or indirect supervision of a Registered Nurse.
HIRING REQUIREMENTS
- High School diploma or equivalent required.
- Current and valid HHA or CNA certification as applicable per state regulations.
- Successful completion and demonstration of competency skills assessment and test under the direction and supervision of a Registered Nurse.
- Current valid CPR certification.
ESSENTIAL RESPONSIBILITIES
- Activities of daily living (ADLs): Personal care activities provided every day, which include bathing, grooming, dressing, eating and hydrating, toileting, ambulating and exercising.
- Measurement and patient observations: Measuring, recording, and reporting client vital signs (temperature, pulse, respiration, and blood pressure), height and weight, intake and output, meal consumption, changes in patient condition.
- Procedures outlined in home care policy and procedure manual and performed under the supervision of a licensed nurse: Performing skills including enemas; applying non-sterile dressings; applying ice packs, attending to skin and oral needs.
- Infection control: Adhering to standard precautions; that is preventing and isolating patient infection through hand washing, care and handling of contaminated objects, isolation procedures, and observing and reporting environmental situations that might spread infection.
- Assisting with patient ambulation, movement, ROM and exercise: Lifting, moving and transporting patients from one position to another, from one room to another, or from wheelchair to bed. Assisting the patient to maintain or regain normal range of motion and body strength and assisting physical exercise to maintain musculoskeletal function and general well-being based on physician order POC.
- Environmental care and safety: Making patients’ living conditions as comfortable and safe as possible by keeping the patient’s rooms clean and tidy and making beds, adjusting room temperature and lighting, removing potential safety hazards or sources of personal injury such as spills or objects left on the floor, performing CPR and assisting patient with evacuation in case of fire or other environmental threats.
- Attend scheduled patient appointments with the patient and parent/guardian to transport the patient to and from scheduled appointments. HHA’s are not permitted to transport patients.
- Managing equipment and patient supplies including restocking of patient supplies. If equipment is not functioning or inventory of patient supplies is needed, notify supervising nurse or Office Clinical Supervisor.
- Communication: Verbal or written communication with patients’ visitors, and healthcare team members; observing and recording patient care, services and/or information.
- Compliance with all Thrive SPC policy and procedures.
- Performs additional duties and responsibilities as deemed necessary.

TRAVEL REQUIREMENTS
- Travel to patient homes and office locations.

While performing the duties of this job, the employee is regularly required to talk and hear. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
This position requires intermittent physical activity, including standing, walking, bending, kneeling, stooping and crouching as well as lifting and supporting patients.
