Position Summary:

Under the supervision of the Social Services Technical Manager, the PACE Social Services Supervisor, oversees site-specific San Diego PACE Social Services provision, consistent with   standards of social work and San Diego PACE, promoting and ensuring quality, efficiency and accessibility of Social Services to PACE participants and families. The Social Services Supervisor   is responsible for leading social workers and case Managers in the provision of social work services and ensuring high quality standards within the Social Service Department. The Social   Services Supervisor may, as needed, carry a case load (panel) and will train social workers and case managers to effectuate social services, including planning, organizing, implementing and   closing the loop for all social services needed for San Diego PACE participants and their families. The PACE Social Services Supervisor will oversee outcomes for the department and provide   weekly quality and productivity reports to the Site Manager and Social Services Technical Manager. Responsibilities include but are not limited to: organization of daily workflow, supervision of   social service employees and related client outcomes, of QAPI projects, assessment, auditing charts and managing employee performance, treatment(s), teaching and counseling to participant, caregiver or other appropriate representatives. Social Services is the liaison between the interdisciplinary team, caregiver representatives, and community agencies.

 

Essential Functions of the Job:

  • Responsible for the overall provision of direct social work and case management services to the participants of the San Diego PACE program, ensuring adherence to policies and procedures of PACE, applicable laws and licensing requirements at all times.
  • Ensures Social Workers actively engage and contribute meaningfully during Interdisciplinary Team meetings
  • Train social workers to conduct robust psychosocial assessments, care planning and periodic re-assessments (minimally every 6 months) of each participant's case. 
  • Participate in the hiring process, train and develops Social Workers and case managers.
  • As assigned, conduct robust psychosocial assessments, care planning and periodic re-assessments (at least every 180 days) of participant's cases. 
  • Ensures assessments, and care plans are thorough and completed prior to the scheduled team meeting, including communicating participant changes to team members.
  • Ensures that all MSWs complete Plan of Care signing and documentation in EPIC
  • Appropriately escalate issues/concerns to Social Service Technical Manager
  • Oversees the development of Social Workers and case managers including monitoring and feedback of all aspects of services provided to individuals.
  • Providing superior customer service to stakeholders, and team members.
  • Ensures resources are available to provide appropriate referrals community agencies or facilities; acts as liaison with such organizations and as an advocate for participants.
  • Consults with, develops and advises staff members as to the interconnectedness of social, emotional and cultural factors, and their impact on health.
  • Fosters a positive and professional environment of effective relationship building amongst staff and maintain a cohesive team.
  • Participates in Program and Policy development of Social Work department.
  • Provides clinical and crisis management support to individuals, Employees, & Providers.
  • Maintains working knowledge of facility service areas including transportation, community characteristics and geography.
  • Ensures that participants receive a visit by appropriate PACE staff member within 24 hours of hospital admissions or on Monday if participant is admitted on Friday or weekend.
  • Coordinate hospital discharge, in conjunction with interdisciplinary team, including communication with attending physician. 
  • Communicates with family, legal representatives and caregivers frequently and as appropriate.
  • Acts as a resource for education and training to other team members and day center staff regarding topics such as dementia, difficult behaviors, and difficult personalities
  • Ensures that all participants complete Care Plan signing after every assessment. 
  • Assigns & monitors Social Workers and Case Manager caseloads for proper service deliver, quality and compliance.
  • Ensures required documentation is in place, accurate, up-to-date, and in compliance with internal and external regulations and policies.
  • Acts as site program administrator during absence of Social Service Technical Manager.
  • Resolves employee issues within position responsibilities.

 

Additional Duties and Responsibilities:

  • May work on special assignments in addition to normal job functions, including traveling to other San Diego PACE sites.
  • Enhances professional growth and development through participation in educational programs current literature, in-service meetings and workshops.

  • Actively participates in the implementation of the  I-SAT and HOS M surveys

 

Job Requirements:

Education Required:

Master's Degree from an accredited School of Social Work 

 

Certifications/Licenses Required:

Valid California driver’s license. Reliable transportation with proper insurance to meet San Diego PACE guidelines. First Aid/CPR certification.

 

Experience and Required Skills:

  • 1 year of experience in a healthcare setting; work must have included social work provisions to frail and/or elderly populations.
  • Counseling theory and techniques and ability to effectively and efficiently utilize EPIC EMR and other documentation outlets.
  • Knowledge of community resources and appropriate utilization to meet the needs of clients and employees.
  • Organization and project management skills.
  • Ability to establish and maintain good interpersonal relationships. Ability to work independently and meet deadlines.
  • Comfort performing home visits to private homes of San Diego PACE applicants and participants with frequent travel in San Diego county.
  • Understanding of Scope of Social Work practice and authority to practice

 

Preferred:

  • Experience as a Clinical Social Worker and/or demonstrated experience as a leader.
  • Demonstrated ability to conduct individual and group therapy for geriatric clients
  • Analytical thinking and problem solving abilities.
  • Able to meet a standardized set of competencies for the Social Services Department, as required by San Diego PACE and approved by CMS before working independently.

 

Verbal and Written Skills Required to Perform the Job:

Excellent verbal and written communication skills. 

 

Equipment Used:

Office equipment, including but not limited to, computers, phones (including

cell phones), copy machines, fax machines.

 

Working Conditions and Physical Requirements:

Prolonged periods of sitting and standing. Frequent walking. Driving within the County of San Diego. Lifting of no more than 15 lbs. Ability to remain calm and objectively evaluate problems. May be required to work outside normal working hours.

 

Universal Requirements:

Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
