Job Summary:
The Foster Care & 29I Liaison will work in collaboration with applicable OLV Human Services staff members to carry out the responsibilities described below in cooperation with the Foster Care Coordinators of the contracted MMCPs (Medicaid Managed Care Plans) in our area. In addition, this position will also be responsible for tasks related to the day to day organization and functioning of the foster care program. These include responsibility for aspects of case documentation for children in foster care, including admission documents, medical record requests, and making contact with collaterals. The Foster Care and 29I Liaison will work within the NYS Connections system, NYS Binti system, and agency Electronic Medical Record to assist program staff in ensuring documentation requirements meet federal, state, county, and agency standards. This position will work in coordination with all program staff to help facilitate the efficient operation of the foster care program.
Essential Job Duties:
1. Coordinate with the MMCP's foster care liaison/coordinator
2. Will attend all meetings with OCFS to discuss enrollment numbers, utilization of services, denials and issues with managed care plans.
3. Inform and coordinate with the MMCP when a child's primary care provider needs to be updated
4. Develop a relationship with each carrier's credentialing or contracting representative and stay up to date on any changes with the carriers
5. Be the primary contact person for the MMCP to assist with the following: MMCP enrollment, disenrollment, and transitions, immediate issuance of a Welcome Letter, securing identification showing the effective enrollment date (including immediate temporary identification) and obtaining replacement insurance identification cards
6. Assist with foster care placement changes including, access to health care and facilitate a single case agreement when a child is placed outside of the MMCP's service area or provider network
7. Interact with clinical and billing staff
8. Maintain records in a safe and secure environment
9. Communicate with providers on any enrollment issues, needs and updates
10. Refer children for needed services and assist in provider selection
11. Coordinate with health care providers, including school- and community-based services
12. Confirm eligibility through Healthenet and MedNY. Scan all documents into charts.
13. Obtain authorizations for families for specialty visits and surgeries
14. Assist with consent and/or confidentiality issues
15. Responsible for completing transmittal forms to be sent to the managed care plans.
16. Set up and manage paper or electronic filing systems for clients, to include recording information, updating paperwork, maintain documents, records and correspondence
17. Create, maintain, enter, and upload information into assigned databases; Connections, Binti and the Electronic Medical Record system
18. Assist program staff with admissions and discharges by submitting admission and discharge Forms to necessary individuals.
19. Create/prepare and send consents, admission documents, correspondence, foster family inquiry packets, applicant decision letters, etc and coordinate with appropriate staff for follow-up on such documents
20. Support Program staff by maintaining files of all necessary Title IV-E documents and distributing to LDSS as requested
21. Develop and maintain placement logs for agency foster homes.
22. Primary responsibility for scheduling 45 day and 6-month file reviews for foster care clients at time of admission.
23. Complete file reviews and prepare discharged foster care client files for closure and storage.
24. Maintain program census in appropriate databases.
25. Primary responsibility for coordinating, accepting, and organizing storage of donations to the foster care program. To include submission for requests of holiday food baskets, holiday gift giving drives, school supplies, suitcases and care cases, clothing, etc. Working in collaboration with the program Directors and staff to ensure distribution of donations to families in need.
26. Support program staff by distributing mail, ordering and dispensing office supplies, and submitting expense reimbursements.
27. Answer telephones, give information to callers, take messages or transfer calls to appropriate individuals.
28. Any other duty as assigned by your Supervisor.

Education:
Minimum Education Preferred: Associate's degree in related field
Experience:
Minimum Experience Preferred: 1 year; Experience in the following areas is a plus:
• The child welfare system
• Foster care healthcare requirements
• The unique complex needs of the foster care population
• Medicaid Managed Care policies and operations

Physical Requirements:
The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required. There is little, if any, exposure to environmental conditions.
