Enrollment Specialist

This is a Full Time Position. Typical schedule is Monday - Friday, 8:00AM - 4:30PM.

Job Summary: Under the supervision of the Enrollment Director, obtains information 
relating to enrollment, coordinates intake and general enrollment processes, and 
provides information and referrals to community agencies as appropriate. 
Promotes the St. Paul’s PACE Program to potential enrollees and community 
agencies.

Duties & Responsibilities:
• Be the main point of contact for assigned potential enrollees
• Ensure that all assigned potential enrollees’ forms and documentation are 
completed and submitted to the appropriate recipients and agencies within 
required deadlines
• Be knowledgeable about St. Paul’s PACE and alternatives to St. Paul’s 
PACE
• Be knowledgeable of other services that are provided by St. Paul’s Senior 
Services and how to transfer someone to those services
• Share the value of St. Paul’s PACE services with potential enrollees 
passionately and frequently
• Complete Intakes in potential enrollees’ homes (including assessing home 
situation and client needs, reviewing client’s medications, taking photos of 
ID’s, insurance cards, and Social Security cards, reviewing and obtaining 
signatures for all intake and insurance forms).
• Refer potential enrollees to other community agencies when needed during 
the PACE Enrollment process.
• Conduct outreach to local community agencies, senior buildings, and senior 
organizations when workload permits
• Document Intakes in EHR and CRM / record keeping tool (e.g., Excel)
• Complete tracking reports and attend trainings as assigned
• Provide process improvement recommendations to other team members 
and leaders
• Miscellaneous tasks given by Management Team and/or colleagues, 
including marketing activities

Qualifications & Requirements:

Education: A Bachelor's degree (preferred) in business, or a health or human services 
field, or a high school diploma and office assistant training (preferably health care 
specific)

Experience: Two years of experience in a marketing, sales and/or admissions role for a 
health care provider, or in an information and referral resource role within the senior 
network. Shall have either training or related experience in the job assigned. A 
minimum of one year of experience working with the frail or elderly population or a 
willingness to receive required training.
