LVN Charge
This is a NOC Part Time Position including 8 hour shifts, 3 days a week. Schedule is 6:00PM - 2:30AM.
BASIC FUNCTION: Acts as a direct supervisor to caregiver staff and supports the Director of Nursing in overall management and oversight of resident care. Ensures provision of quality nursing care following all federal, state and local rules and regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Directs, evaluates and counsels caregiver staff on job performance and expectations. Identifies areas of improvement and develops improvement plan for assigned staff. Recognizes staff for accomplishments and good job performance.
- Evaluations and supports care staff in providing safe, compassionate individualized care to residents. Provides regular feedback and instruction to care staff pertaining to resident health status, and care needs.
- Regularly attends manager/supervisor training and meetings.
- Assists in administration of medication to residents as prescribed by the physician and following all St. Paul’s policies and protocols.
- Performs Resident assessment as condition warrants. Initiates and updates care plans as needed Reviews and compares physician’s orders with Resident’s plan of care to assure appropriate implementation.
- Observes, documents, and communicates to department director any and all changes in Resident’s physical and mental capacities. Provides information for weekly Resident evaluation meetings.
- Notes and executes physician’s orders correctly and as required.
- Assists Nursing Director in the orientation and in-service of Nursing Department staff.
- Identifies safety hazards and takes action to ensure correction as required and in a timely manner.
- Assesses Resident equipment and supply needs and makes recommendations to the physician and/or the family. Communicates with on-coming shift regarding Resident care needs.
- Completes reports and assignments accurately and adheres to established time schedules and frequencies.
- Assists in the ordering/supervision of medical supplies.
- Establishes and maintains open lines of communication between all Nursing Department personnel.
- Assists in the interviewing, hiring and counseling of Nursing Department staff members.
- Completes CNA performance evaluations for 90 day and annual evaluations.
- Approve payroll hours for assigned CNAs.
- Ensures the documentation of the care and treatment provided to Residents and the Resident’s response or lack of response to the care provided.
- Adheres to all St. Paul’s policies and procedures for the admission, discharge, and transfer of Residents. Complete charts on all Residents accurately and timely, and follows all infection control procedures. Ensures full compliance with all Resident’s Rights requirements.
- Works cooperatively with Residents, supervisors, employees, physicians, families, consultant personnel and ancillary service providers.
MINIMUM QUALIFICATIONS: Must have a current California LVN license and have current CPR certificate. Must be interested in and have compassion for the elderly. Must possess good communication skills, both verbal and written and be fluent in English language. Needs to have organizational skills sufficient to allow for meeting deadlines and goals in a dynamic environment. Must be computer literate sufficient to perform all tasks required of the Electronic Medical Records (EMR) System.
EDUCATION AND EXPERIENCE: Graduation from an accredited School of Nursing with minimum of one-year experience in direct/supervisory care of the elderly, preferably in an assisted living/long-term care facility.
