Case Manager I
The Colorado Coalition for the Homeless seeks a Case Manager I to provide comprehensive case management services to American Indian and Alaska Native (AI/AN) individuals, households, and families who are experiencing homelessness in Metro Denver, with the overall goal of decreasing health disparities and increasing health and wellbeing among this population. Principal focus is linking participants of CCH Talking Circle ceremonies and other Native American Services clients with integrated primary and behavioral health care services provided at CCH's Stout Street Health Center (SSHC). To address the spectrum of social determinants of health, additional services provided include but are not limited to: crises intervention, provision of basic needs (socks, food, cold weather gear etc.), facilitation of increased access to emergency shelter, transportation, client advocacy, and linkage to legal services, public benefits, identification, substance treatment, and housing. This position is located at 2100 Broadway, Denver, Colorado 80205 and reports to the DOES Program Manager.
NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions
- Assists with directly connecting American Indian and Alaska Native (AI/AN) individuals, households, and families experiencing homelessness to integrated care services at the Stout Street Health Center, including but not limited to: setting appointments, accompanying clients to appointments, coordinating follow up care with medical staff, and consulting regularly with client’s healthcare team to update treatment plans or modify interventions, as needed.
- Works collaboratively within CCH, especially with providers at the Stout Street Health Center and colleagues in the Department of Outreach and Engagement Services, and externally with homeless service providers to maintain optimum care coordination, communication, and continuity of services for clients. Accepts appropriate referrals from CCH staff and other agencies, especially other Native-led and Native-Serving Organizations such as the Denver Indian Center, Denver Indian Family Resource Center, Denver Indian Health and Family Services, and 4 Winds Native American Council.
- Provides comprehensive case management services outside of the office environment through street outreach. Engages and obtains outcomes in a trauma-informed and culturally competent manner to clients of the Native American Services program, focusing on health, wellbeing, and long-term stabilization.
- Obtains services and materials to assist clients off the streets and into treatment, housing and/or employment, including birth certificates, identification, benefits acquisition, substance treatment facilities, and housing applications.
- Completes all necessary documentation and electronic data entry requirements in HMIS and EHR in a timely and thorough manner and in compliance with all relevant HIPAA regulations. Maintains client confidentiality.
- Assists with and participates in programmatic aspects of Native American Services when appropriate, including Red Road to Wellbriety recovering programming, Wiping of the Tears, and seasonal celebrations. Is available after Talking Circles to answer general resource questions and make case management connections and referrals to participants.
- Advocates for the needs of individual clients as well as systemic change, whenever needed for the overall benefit of persons who are homeless. Participates when appropriate in inter-agency meetings focused on the health and housing disparities of AI/AN persons, including the Native American Housing Circle and the Seven Sisters.
- Makes an average of 5 documented contacts (or more) with clients per day. Treats each person contacted with dignity and respect regardless of negative or positive client behavior.
- Transports individuals who are homeless using a personal or organizational vehicle.
- Meets with the media, students and other persons interested in the issue of homelessness for educational, funding and advocacy purposes.
- Participates in the Point in Time Survey, VI-SPDAT assessment, and the Client Satisfaction Survey.
- Is available by cell phone during all assigned shifts.
- Returns all e-mails and voicemails.
- Record all services in a timely and complete fashion in the EHR.
- Maintain up to date records in EHR.
- Record telephone and other communications as needed in the EHR.
- Manage tasks and scanning as needed to have a complete and current chart.
- Maintain accurate information regarding client’s insurance status.
- Utilize the EHR and any future upgrades as directed.

Qualifications Summary
- Bachelor’s degree in human services, social work, psychology, international relations, or related field. Experience may be substituted for education.
- Significant case management experience or direct service work with American Indians/ Alaska Natives preferred.
- Previous direct experience with homeless population preferred, or previous experience in a related direct human service field working with underserved populations preferred.
