Recuperative Care - Registered Nurse/ Licensed Vocational Nurse
We are seeking a skilled Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to join our Recuperative Care Program (RCP) Team at the 40 Prado Homeless Services Center. As an RCP Nurse, you will play a crucial role in monitoring the safety and stability of our participants. You will coordinate services with the RCP Case Manager, staff, and other professionals, including home health and on-site Community Health Center (CHC) medical staff, to ensure continuity of care and a smooth transition back into the community. Your work will directly support CAPSLO’s mission of helping individuals achieve self-sufficiency.
We would love to hear from someone who:
- Is a California licensed Registered Nurse, or a licensed Vocational Nurse.
- Is CPR certified.
- Has two years experience in a hospital setting.
- Has experience working with those experiencing homelessness.
- Has a valid California Driver’s License and proof of insurance.
As the Nurse you will:
- Monitor the safety and stability of participants.
- Coordinate with RCP Case Manager and medical staff to ensure continuity of care.
- Facilitate the transfer of individuals from hospitals to 40 Prado.
- Provide education on health and lifestyle issues, including nutrition, physical activity, and substance use.
- Advocate for participants’ health needs and ensure continuity of treatment.
- Detect and report signs of depression, abuse, or neglect.
- Train RCP staff on medical and medication-related issues.
- Accompany participants to medical appointments as needed.
- Collect and manage data to support participant care.
Additional Eligibility Qualifications:
- Must be able to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, veteran status, sexual orientation, disability or socio-economic level.
- Must be at least 18 years of age by date of hire.
- Must comply with the CAPSLO COVID-19 Vaccination Policy, either being fully vaccinated or sign a COVID-19 vaccination declination form.
- Based on program, a current TB skin test/Chest X-Ray may be required.
- Employment is contingent upon meeting all applicable job requirements and background requirements. A criminal history clearance is required prior to employment.
- Upon hire, agency staff will check the Megan’s Law (or the current sexual predator website) to ensure each staff being hired is clear for employment. Agency staff will check the website annually to ensure all program staff are cleared to work.
