Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI provides residential, outpatient, inpatient, patient-centered medical homes, medication-assisted treatment, and crisis services to individuals experiencing crisis, opioid use disorder, homelessness, and mental illness.

Job Summary
The Outreach Navigator serves a population of pregnant and parenting clients as well as those with an SMI designation. Their primary role is to enhance the likelihood of success, identify and reduce barriers and connect them with needed services and resources, including COVID-19 Personal Protective Equipment (PPE) supplies and education. The Outreach Navigator will work with and outreach pregnant and parenting individuals and SMI clients in need of additional support and provide case management and ongoing wrap-around supportive services. Services include COVID-19 PPE supplies, and COVID-19 education and resources. The Outreach Navigator will provide services to individuals pre/post incarceration, as well as individuals with a recent Crisis Mobile Team activation. The Outreach Navigator maps out the most appropriate next steps for individuals in need of assistance with service access. The Outreach Navigator is part of a multi-disciplinary team made up of internal and external stakeholders. The Outreach Navigator will document, and track required patient interactions according to the Arizona Administrative Code (AAC), Arizona Department of Health Services/Department of Behavioral Health Services (ADHS/DBHS) Provider Manual, Commission on Accreditation of Rehabilitation Facilities (CARF), Community Bridges Policies and Procedures, and specific for the program.
Skills/Requirements
• Highschool diploma or GED and either:
o 18 credit hours of post-high school education in a field related to behavioral health and two years of full-time behavioral health work experience; or
o four years of full-time behavioral health work experience.
• Has an associate’s degree and at least two years of full-time behavioral health work experience; or
• Has a bachelor’s degree in a field other than behavioral health and at least one year of full-time behavioral health work experience; or
• Has a master’s degree or bachelor’s degree in a field related to behavioral health.
• Minimum of ninety (90) days experience in a Navigator or Peer Support Specialist role required.
• Minimum of six months of recovery from substance use and/or mental health disorders required.
• Current AZ Driver's License (valid and in good standing).
• Clear 39-month Motor Vehicle Record.
• Arizona Fingerprint Clearance card (program specific)
