THE ROLE: The PT Case Manager (18.75 hours per week) will provide direct services to the Aging and Independent Living program participants. Participants are of all ages and can be in need of multiple types of services. Essential duties may include but are not limited to: 

ESSENTIAL DUTIES:

· Outreach communities, individual entities and businesses to inform them of program and program opportunities. Convey the purposes and services of the program being represented to the user population and the impact that program or service would have on their health and wellbeing.

· Develop and maintain relationships with communities, resettlement agencies, mutual assistance associations, and other community-based organizations, and participate in the service networks as necessary.

· Work effectively with community members (staff, clients, doctors, agencies, etc.) to reduce cultural and socio-economic barriers between clients and institutions.

· Provide culturally and linguistically appropriate case management services to qualified program participants.

· Accept client referrals as directed by each program and manage response times as required per program.

· Assess, and address needs as presented and determined by individual program assessment requirements.

· Resource Navigation: Assist individuals in obtaining appropriate resources and connect to applicable support groups that may include workshops/educational resources, respite care services, referrals to state and federal benefits, nutrition programs, family caregiver programs, kinship care, care coordination, Medicaid case management and/or other services available.

· Maintain contact, follow up and discharge clients as outlined in individual programs.

· Ensure appropriate documentation and gather outcome data under the direction of the Program Supervisor. Document in applicable program system as needed.

· Coordinate and maintain contact with team members to foster collaboration, productivity, creativity and help in identifying and implementing solutions.

· Adhere to all LCSNW and contractor guidelines, policies and procedures.

· Attend, participate and complete all required trainings, meetings and internal/external gatherings.

· Communicate information to clients and providers clearly and succinctly, both written and verbal. Assist with language and interpretation needs as necessary

· Identify gaps in services and report to the Program Supervisor

· Requires strict adherence of HIPAA standards to ensure confidentiality.

· Perform other duties as assigned.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

· Bachelor’s degree or equivalent work experience in relevant field.

· Minimum of 2 year working in a case management or social service role.

· Minimum of 2 year working with diverse communities of all ages.

LANGUAGE SKILLS: Ability to read, write, and understand directions. Must have the ability to write memos and other basic correspondence. Ability to effectively present information and respond to questions from employees, clients, and the general public. Must have excellent communication skills with the ability to read, write, and speak English.
