SSVF Health Care & Benefits Navigator

The Health Care & Benefits Navigator will collaborate with project staff to ensure Veterans experiencing homelessness have access to health care, mental health services, and benefits. This role involves creating partnerships with community health providers, mental health providers, and Veterans Administration Health Care Providers, as well as other relevant agencies. The Navigator will act as a liaison to help Veterans understand their health care plans and benefits, and assist with filing claims. Additionally, the Navigator will be a resource for staff on health care options and benefits programs, supporting CAPSLO’s mission to help Veterans achieve stability and self-sufficiency.
Qualifications:
- Has a dependable, insured transportation and valid California driver’s license (mileage to be reimbursed) and acceptable driving record. A DMV printout and proof of insurance will be required.
- Bachelor's Degree in Social Work, Psychology, Family Studies, Human Services, or a related medical field, or an equivalent combination of education and experience in coordinating medical or mental health services, totaling four years.
- Preferred: Master’s level social worker or equivalent education and experience.
- Experience working with individuals or families, Veterans, the mentally ill, or the chronically disabled who are experiencing homelessness.
- Experience providing education, referral, and care coordination services.
- Veterans with other than dishonorable discharge preferred.
As the SSVF Health Care & Benefits Navigator you will:
- Case Management: Provide case management, care coordination, and health education. Conduct assessments to identify and address barriers to care. Assist Veterans in communicating their health goals.
- Support and Education: Serve as a resource for Veterans and their families, helping them find appropriate resources. Collaborate with providers to reassess health care needs and address systemic barriers.
- Benefits Navigation: Assess eligibility for government benefit programs, assist with applications, and advocate for Veterans. Follow up on documentation and communicate claim statuses.
- Client Management: Maintain professional relationships, monitor compliance, and ensure confidentiality. Complete required documentation and enter data into the HMIS.
- Additional Duties: Develop care plans, assist in policy development, build community relationships, and adhere to ethical principles and safety rules. Perform other duties as needed.
Additional Eligibility Qualifications:
- Must be able to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, veteran status, sexual orientation, disability or socio-economic level.
- Must be at least 18 years of age by date of hire.
- Must comply with the CAPSLO COVID-19 Vaccination Policy, either being fully vaccinated or sign a COVID-19 vaccination declination form.
- Based on the program, a current TB skin test/Chest X-Ray may be required.
- Employment is contingent upon meeting all applicable job requirements and background requirements. A criminal history clearance is required prior to employment.
- Upon hire, agency staff will check the Megan’s Law (or the current sexual predator website) to ensure each staff being hired is clear for employment. Agency staff will check the website annually to ensure all program staff are cleared to work.
