Community Health Worker - Mobile Van

POSITION SUMMARY
The Community Health Worker will function as a member of the Mobile Medical Team and support services and activities on Wheeler’s Mobile Medical Unit(s). The Community Health Worker will build community partnerships and schedule events for Wheeler’s Mobile Medical Unit (s) and provides education, outreach and support to the community through the Mobile Medical Unit(s).The Community Health Worker works within the multi-disciplinary team to reduce disparities to care, supports individuals and families with specific disease management, increase access to primary care, behavioral health and community resources. Wheeler Health & Wellness provides an integrated, comprehensive continuum of patient-centered primary care, behavioral health and community resources to enhance health equity and achieve the triple aim of health reform: improve health, reduce costs & increase quality. This position will support activities and events at and associated with all Health and Wellness Centers as requested/needed.

Hourly Rate $20.00
EDUCATION AND EXPERIENCE/QUALIFICATIONS
High School Education or GED plus 2 years of experience working with individuals and families in the community are required. A valid Connecticut Driver’s license is required, as staff will function as a member of the Mobile Medical Team. Must also have access to reliable transportation. Must have held driver’s license for a minimum of one year.
Ability to work on a team to ensure program goals are met, communicate effectively verbally and in writing, and is committed to personal and program growth. Ability to manage time effectively. Ability to represent the agency in a professional manner.
LOCATION
Office location will be Plainville, CT, however position requires travel to scheduled events, as well as other Wheeler health center locations.
SCHEDULE
Full time. Hours will be based on needs of mobile operations, but some flexibility may be possible outside of scheduled activities/events. Some weekend hours may be required.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Participates in the planning, coordination, and implementation of community engagement, education, outreach and support activities, and primary care and behavioral health services through Wheeler’s Mobile Medical Unit(s).Outreach and education activities include prevention, health promotion, and wellness training events and programs; health screening events; recovery support services; mutual support groups; and referrals to other community resources.
Responsible for safely driving and operating the Mobile Medical Unit, including all associated operations and maintenance tasks. Tasks may include but are not limited to operating and/or maintaining the unit’s slide, leveling system, propane, gas, waste and water systems.
Oversee that all necessary supplies and equipment are organized and loaded on and off the mobile medical unit to ensure it is stocked for events. Supplies and equipment may include scales, vitals cart, AED, point of care testing, IT equipment, promotional items, and paperwork for registration.
Creates new patient/client charts as needed, including but not limited to demographic and insurance information and obtaining all required patient/client signatures. Reviews patient informed consent and rights & responsibilities with patients.
Collects vitals including height, weight, blood pressure and temperature. Performs toxicology screens with patients and accurately reads results.
Conducts/collects questionnaire data, e.g., PHQ 9, PRAPARE, DLA-20, OHIO, CAGE, ACE
Provides peer support and role modeling in the Mobile Medical Unit(s), office, and/or community settings outside of the office to teach families how to identify needed resources and supports in their communities.
Supports individuals and/or families in the education of specific disease management
Supports daily program specific operational functions as required. May transport clients to appointments or linkages as appropriate. May involve visiting clients in their homes and/or community settings
Attends and is prepared for scheduled supervision, team meetings, staff meetings or rounds. Seeks additional supervision or consultation as needed and follows through with supervisory directives.
Demonstrates clear, concise, professional writing and oral reporting skills with basic computer skills.
Works effectively as a member of a multi-disciplinary team by communicating appropriately and positively with colleagues, families, internal and external service providers, administrative staff and actively preparing for and participating in team meetings.
Maintains competencies to meet expectations in all position-specific key performance indicators.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
