Fee for Service Bilingual Outpatient Clinician
POSITION SUMMARY
The Fee For Service - Bilingual Integrated Health Care Clinician provides clinical and case management services for children and adults across the lifespan in an integrated health care setting that provides primary care, behavioral health and dental services. The Integrated Health Care Clinician will foster positive working relationships with co-workers, clients and external key stakeholders. The Integrated Health Care Clinician provides leadership, motivation and support to staff, manages the referrals, admission and discharge processes for clients, and ensures the integrity of the program model.
EDUCATION AND EXPERIENCE/QUALIFICATIONS
Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field or a doctoral degree in Psychology is required.

Licensure Requirements for Clinical Positions
Must hold one of the following credentials: LMSW, LCSW, LMFTA, LMFT, LPCA, LPC or licensed or license-eligible Psychologist in the State of Connecticut.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Works as part of a multidisciplinary team to efficiently and effectively meet the behavioral, primary, and dental health needs of the client, including, but not limited to: providing behavioral health consultation to the primary care provider, assisting client in meeting medical treatment compliance, and referring client to other Wheeler services and/or other community services as needed.
Conducts program specific screening and evaluation, and formulates diagnosis in accordance with the Diagnostic Statistical Manual (DSM). Develops and documents, clear clinical formulations and treatment plans that are individualized and measurable, developed in collaboration with the client/family and others as relevant, using behavioral health screening as directed, and are inclusive of behavioral health, primary care, and dental goals as indicated.
Demonstrates knowledge of psychopathology and other behavioral health concerns and primary modalities of psychotherapeutic intervention. Triages clinical emergencies and implements program specific safety and consultation protocols. Utilizes best practices and evidenced based practice models where appropriate and adheres to integrated health model and program expectations.
Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for identified service needs in a timely manner.
Communicates in a positive, effective manner with client, family, and all internal and external providers. Attends any required meetings and multidisciplinary treatment team planning meeting.
Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols.
Documents all client related information in compliance with agency policy.
Attends and is prepared for scheduled supervision. Seeks additional supervision or consultation as needed and follows through with supervisory directives.
Provides clinical leadership by implementing, promoting, and sustaining evidenced based and best practice models of prevention, intervention, and treatment.
Provides guidance and crisis support to staff.
Works with program manager or designee to develop, monitor, and analyze clinical and program performance and outcomes and develops and monitors quality improvement program goals.
Interfaces and collaborates with and other community providers to provide positive system development for clients and families, to promote Wheeler Clinic's reputation as a quality service provider.
Serves as role model for staff to promote strong work ethic and adherence to model specific approaches.
Ensures that staff provides culturally competent care and maintains sensitivity to and respect for clients' cultural traditions, values, and beliefs.
Provides training and technical assistance to clinicians on utilization of the electronic health record.
Reports child abuse/neglect as mandated, adheres to confidentiality guidelines and ensures that the program meets all necessary licensing, regulatory, and accreditation standards, as related to the program, staff, records, facility, and care of youth.
Oversees the orientation of new clinician staff.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
