Registered Nurse (RN)
Community Youth Services - Framingham
Wayside Youth & Family Support Network's Framingham Site is seeking Full Time (40 hours/week) Youth Nurse (RN) for an exciting opportunity to join an innovative, evidenced based model of treatment in Massachusetts, the Program for Assertive Community Treatment for Youth (PACT-Y). This program is a DMH funded, intensive, self-contained, community-based program, serving youth and their families through an integrated, interdisciplinary team. This position offers a diverse caseload, working with youth and caregivers in the home and requires flexible hours that include at least 2 evenings a week, Monday – Friday.
Salary Range: $78,000.00 to $83,000.00/year + sign-on bonus (half paid after 30 days of employment and half paid after 90 days of employment; pro-rated for part-time)
What You Will Do:
• Work on a cohesive, interdisciplinary team to support youth and young adults in their homes and community.
• Coordinate care to support the overall well-being of the youth including their medical and emotional needs.
• Oversee and implement the medical, health and medications services of youth in the program inclusive but not limited to:
- Triage calls - Medical concerns vs. psychiatric medication refills
- Pharmacy communication
- Prior authorization
- Coordination of services with psychiatry and primary care
- Help coordinate care for chronic conditions
- Administer injectables
- Wellness visits: education on nutrition and exercise
- Help set up follow-up visits for clients not adhering to the treatment plan
- Ensure labs are completed and entered in Quest
- Coordination with school nurse: complete forms
- Help facilitate obtaining medical records
• Maintain a Resiliency-Based approach through all aspects of work.
• Maintain communication with youths’ legal guardians and providers for all medical issues on a continual basis.
• Inform team and families on essential information related to each youth’s medication, health and medical history.
• Participate in all necessary meetings, including site staff meetings, weekly team consultations and daily check-ins.
• Maintain compliance with all agency and state policies, standards and protocols, including all pertinent documentation.
• Along with other staff, take turn covering weekly on-call; compensated per on-call rotation
• Maintain First Aid and CPR certification
• Participate in Wayside nursing meeting supporting each other and facilitating client care
• Be part of the agency nursing team to support program functions

Minimum requirements:
• Must have a current, unrestricted license as a Registered Nurse in Massachusetts.
• Have experience working with youth in a psychiatric environment.
• Driving in the community is a requirement of this position; please ensure you have a valid driver's license and access to a reliable vehicle.
We will train you in: Motivational Interviewing; Courageous Conversations; Attachment, Regulation and Competency Model (ARC); Trauma Informed Practice; Substance Use Intervention; Risk and Safety Assessment.
