CASE MANAGER / TRANSITION FACILITATOR


Young Adults Residential - Methuen

 

Wayside's Young Adult Program, Merrimack Valley Young Adult Program is looking for full-time, compassionate and dedicated Transition Facilitator to provide case management of 5-7 assigned clients in the community (program apartments or in-home), outreach to clients living in the community, assessment of individuals abilities with regard to transportation, medication administration, health/wellness, budgeting and substance abuse and development of treatment plans around relevant needs. The Transition Facilitator also provides support to clients who live in the staffed apartment in Methuen.
 
Salary Range: Starting $22.00/hour + sign-on bonus (half paid after 30 days of employment and the other half paid after 90 days of employment/pro-rated for part-time).  

We are looking for the following schedule coverage: Tuesdays to Saturdays.

  
 What you will do:   

  • Coordinate intake assessments for new participants.
  • Schedule weekly case management meetings with assigned participants.
  • Develop, implement, and evaluate service/participation plans in collaboration with participants.
  • Provide individual and group life skills assessment and education/training for participants.
  • Provide employment assistance, career development and apprentice job matching for identified participants. Develop, monitor, and evaluate apprenticeship experiences.
  • Develop and maintain a broad network of community-based support services, including health, employment, education, counseling, self-help, community service, etc.
  • Provide counseling, crisis intervention, referrals, service linkage and advocacy on behalf of program participants.
  • Provide appropriate level of monitoring and supervision for program participants.
  • Complete all documentation as required.
  • Participate in all weekly and required agency staff meetings, required agency trainings, supervision with Program Director and assigned meetings in the community.
  • Assist with the development and implementation of appropriate policies and procedures.
  • Ensure compliance with Wayside Youth and Family Support Network policies and procedures.

 Minimum Requirements:    

  • Bachelor’s degree in counseling, psychology or related field preferred.
  • Experience working in a community-based setting and/or residential setting with youth is preferred.
  • Valid driver’s license and reliable transportation required.
  • Bilingual/Bicultural candidates are strongly encouraged to apply. 

We will train you in: Motivational Interviewing; Attachment, Regulation and Competency Model (ARC); Courageous Conversations; CPR and First Aid.
