Emergency Beds Services Program Coordinator

Pay Range: $21.42 - $24.10
About this opportunity:
- The Emergency Beds Services Coordinator is responsible for the intake process, bed assignment, orientation, case management, and daily operations of the emergency overnight beds program. This position includes assessing client needs, coordinating intake, managing bed assignments, providing case management services, and ensuring the program runs smoothly. The coordinator will also collaborate with the Jacksonville Fire Rescue Department (JFRD) and work directly with individuals in crisis to meet their immediate needs while connecting them to long-term housing and support services
Key responsibilities:
Client Outreach and Intake:
- Manage all referral contacts received from JFRD.
- Conduct initial intake interviews to assess eligibility for the emergency beds program.
- Establish eligibility for services, including possible transition to the Transitional Housing program.
- Explain program guidelines, policies, and procedures to clients during intake.
- Enter intake data into the Homeless Management Information System (HMIS) for accurate client records.
- Coordinate with the team to assign clients to case managers for follow-up services.
Case Management and Support:
- Develop individualized plans based on client needs (e.g., mental health, addiction, housing).
- Provide counseling, support, and referrals to ensure client needs are met, including income, benefits, and psychosocial services.
- Track and update client progress in HMIS to maintain accurate documentation.
- Assist clients with finding permanent housing and accessing supportive services.
Program Coordination and Bed Management:
- Maintain bed rosters, schedules, and entry dates for the emergency beds program.
- Monitor bed occupancy and ensure clients are assigned to available beds.
- Conduct inventory of client possessions, ensuring compliance with program guidelines and behavior expectations.
External Collaboration:
- Build and maintain professional relationships with external agencies and partners to support client needs.
- Facilitate referrals to external services, including healthcare, housing, and addiction recovery programs.
- Work with external landlords, property managers, and housing agencies to help clients transition to permanent housing.
Administrative and Reporting:
- Maintain accurate and confidential client records, ensuring compliance with legal and ethical standards.
- Prepare case reports, progress summaries, and discharge plans as needed.
- Ensure timely completion of reports and other administrative tasks related to program management.
KNOWLEDGE, SKILLS AND ABILITIES:
- Strong understanding of Salvation Army Northeast Florida programs.
- Ability to manage complex caseloads and perform crisis intervention.
- Experience with case management software (e.g., Service Point, HMIS) and proficiency in MS Office.
- Strong organizational, communication, and interpersonal skills.
- Ability to maintain professional relationships with clients, partners, and community agencies.
- Knowledge of program rules, policies, and regulations, and the ability to apply them consistently.
OTHER RESPONSIBILITIES:
- Participate in team meetings and client reviews to assess the suitability of clients for the emergency beds program.
- Respond to emergencies, urgent client needs, and disaster relief tasks as directed by the supervisor.
- Participate in community outreach and educational activities to promote program services and recruit clients
What we are looking for in you:
- Bachelor’s degree in social work, Behavioral Science, or a related field, OR
- Three years of progressively responsible experience providing case management services, including client needs assessments and development of long-term action plans.
- Any equivalent combination of training and experience that provides the required knowledge, skills, and abilities.
LICENSES AND CERTIFICATIONS:
- Valid Driver’s License.
WORKING CONDITIONS:
Works is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
