Case Management Specialist
Location: The Salvation Army St Petersburg Area Command, St Petersburg, FL
Position Type: Regular Full-Time at 40 hrs/wk
Pay Range: $20.50/hour
About this opportunity:
This position is responsible for interviewing, accepting, and providing comprehensive, long-term, structured, complex case management services for an assigned caseload of clients from the community and clients participating in the Pathway of Hope program.
Key responsibilities:
- Interviews applicants to determine eligibility for The Salvation Army services or program enrollment based on established guidelines and requirements; conduct needs assessment, obtain pertinent information, and establish program goals for eligible clients; refers applicants to other agencies, if not appropriate for The Salvation Army services or programs.
- Develops intake and case plans for client; instructs clients in completing necessary paperwork including goals and/or budget forms; completes orientation check-off form and obtains release of information forms when appropriate; completes social history on new clients each week.
- Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same.
What we are looking for in you:
- Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field, AND
- Three years progressively responsible experience providing direct case management social services including accessing clients’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources, OR
- Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities
- Valid State Driver’s License
- Must be able to pass a Level II Background screening.
Physical Requirements and Working Conditions:
Work is performed in a normal office environment where there are little or no physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like.
