The Program Director plans, directs, coordinates, and monitors the day to day operations of a Supportive Housing Program, under the direction of the Director of Social Services; provides supervision for Case Managers, Receptionists, and the Security team. manages and monitors assigned caseloads of clients seeking social service assistance, carries a caseload of four households; develops program plan/goals and evaluates client’s progress by conducting routine meetings with client or staff; serves as an advocate for client in order to acquire services that will enable them to functionally cope with their environment; Ensures program is administered with established policies and procedures and budget constraints; Builds and maintains effective working relationships with community agencies in order to make appropriate client referrals. Assists Director of Social Services with securing grant funding for the Supportive Housing Program.
What we are looking for in you:
Bachelor’s degree from an accredited college or university in Social Work, Human Services, or a related social services field,
And
Two years’ experience performing progressively responsible social service case management work,
Or
Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Job Responsibilities:
Supervises and monitors the day to day activities of the Supportive Housing program as it pertains to housing (attendance, processes and procedures), meal schedule, attendance in life skills classes, transportation, financial assistance, food/clothing/furniture assistance, employment assistance, educational and vocational assistance; ensures that operations run smoothly and are in compliance with the federal government, United Way, other funding relationships, and The Salvation Army policies and procedures.
Plans, develops, coordinates, implements, and evaluates program services based on the changing needs of the clientele; develops programs to assist in the stabilization of homeless individuals and families as well as to provide crisis intervention; conducts assessments to identify program’s strengths and weaknesses and to specify program goals.
Coordinates and monitors the maintenance of program facilities; ensures that the building is properly cleaned, maintained, and secured; coordinates needed repairs in a timely manner.
Conducts team meetings and clinical staff reviews of residents in order to ensure that the needs of the residents are met, and goals are being accomplished.
Presents cases to team, confers with team members, and provides and/or receives information as required in order to reach consensual decisions regarding disposition of cases.
Manages the HMIS project for the Homeless Programs at Area Command, analyzes data monthly and provides monthly programmatic progress to Supervisor.
Assists with the Annual Progress report to HUD and United Way; assists with Annual Supportive Housing Program Grant (HUD) preparations.
Attends monthly meetings within the community, participate in community committees and Advisory Board meetings as requested.
Submits monthly statistical information, time sheets and resident information for program evaluation; prepares and maintains statistical records on all services provided; compiles and prepares monthly statistical reports; ensures the accuracy and completeness of the same. Analyze reports, data, timesheets, and provide regular updates to Supervisor and Development team.
Assists in securing program funding through grants and other proposals. Monitor the speed of Rapid Rehousing funds to ensure that grant spending is complete.
Monitor, review, and advise interns and volunteers.
Confers with other Salvation Army employees to ensure coordination of services, including children’s programs, childcare, operations, building maintenance, volunteers, etc.
Reviews, monitors and implements program policy and procedure; administers program in compliance with budget constraints.
Updates programmatic documents and conduct monthly chart monitoring.
Supervises program staff responsible for providing case management, social services, and shelter assistance; instructs and trains employees in the proper methods and procedures for conducting work; monitors work in progress and upon completion for compliance with established program policies and procedures; conducts performance evaluations and makes salary and hiring/termination recommendations.
Partners with the Director of Social Services to develop staff development programs such as in-service training, seminars, etc.
Responsible for monthly Nashville Area Command calendar of events and meeting room utilizations.
Case Management: (20%)
Interviews applicants to determine eligibility for program enrollment based on established guidelines and requirements; conduct needs assessments, obtain pertinent information and establish program goals for eligible clients, refers applicants to other agencies if not appropriate for program.
Develops intake and case plans for client; instructs clients in completing necessary paperwork including goals and/or budget forms; completes social history on new clients each week.
Conducts an orientation session with new clients, explains the program, guidelines, tiers and expectations.
Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same; ensures that case files are documented appropriately and kept in a workable system, meeting HUD and United Way criteria.
Meets weekly with clients to discuss and evaluate their progress; prepares accurate and up-to-date records documenting the same; receives incident reports and enforces disciplinary actions for infractions; collect fees from clients.
Arranges life skills classes that will assist clients to functionally cope with their environment; obtains speakers and workshop presenters as needed, and devise a monthly calendar of events, in efforts to meet client needs. Ensure a robust life skills educational schedule for supportive housing participants.
Assists clients in locating/securing affordable housing; conducts home visitations when appropriate; ensures that client’s living quarters are properly furnished and maintained; contacts appropriate person to facilitate needed repairs; conducts routine inspections of living quarters and grounds.
Refers clients to appropriate Corps programs or other agencies based on clients’ needs and in accordance with their program plan.
Conducts room inspections at least twice per week and communicates with the Property Department after each inspection.
Completes discharge plans of clients existing the program; transfers file contents to non-active file; prepares card for follow-up mailing.
Reviews case assessments for each client including a personalized action plan; reviews case files to ensure proper documentation of all case activities.
Assumes weekly direct practice caseload. Client centered responsibilities include assessment, the creation of a service plan, referrals, and linking the client to other needed resources; meets with clients on a regularly scheduled basis for service planning, progress monitoring, and timely interventions. Develop and secure partnerships for youth to include; counseling, therapy, and tutoring.
Community Outreach: (20%)
Coordinates with other community resources.
Works with the Volunteer Coordinator in determining need for and placement of volunteers and interns.
Assists in developing program proposals for funders.
Collaborates, collects, and acknowledges donations (linens, blankets, clothes, shoes, games, books, baby food, diapers, etc.)
Represents The Salvation Army at relevant meetings, presentations, etc.
Attends community meetings with representatives of other agencies in order to educate them about the program, make client referrals and maintain awareness of client’s progress.
Other Responsibilities: (5%)
Serves on-call twenty-four hours a day to respond to program emergencies.
Conducts tours of program facility from outside agencies as requested.
Conduct drug screens on residents with drug related issues on a regular basis.
Co-leads Town Hall meetings at least six times a year by delegating responsibilities to residents informing residents of issues, leading problem-solving discussions and forming sub-committees,
Assists with community services as needed.
Assists in performing social service work for special or seasonal projects, occasionally on weekends.
Assists in preparing quarterly and annual reports to United Way and other funding sources.
Performs other related work as required.
Knowledge Skills and Abilities:

Ability to develop program plans and goals based on client's needs.
Ability to evaluate the client's progress toward program goals.
Ability to work with the public encompassing all types of behaviors
Ability to interview clients and to obtain pertinent information.
Ability to prepare and maintain accurate and complete case notes and client records.
Ability to plan, organize, and prioritize work in order to accomplish work in compliance with quality standards and deadlines.
Ability to build and maintain effective and professional working relationships with clients and community agencies.
Physical Requirements and Working Conditions:
Ability to meet attendance requirements.
Ability to read, write, and communicate the English language.
Ability to communicate clearly and effectively both orally and in writing.
Ability to think clearly and quickly in order to maintain control of client caseload.
Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized.
Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) 5-10% of work time.
Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking.
