Social Services & Case Worker

The Social Service Worker provides emergency and non-emergency direct assistance to eligible clients (resident and non resident) for food, clothing, household items, as well as financial assistance with rent and utilities, etc.; researches and provides client referrals to county resources; interviews clients to determine need and eligibility to receive services; prepares accurate and complete client records; refers clients to community agencies and maintains effective working relationships with the same; ensures compliance with program policies and procedures.
Key responsibilities:
Greets and interviews clients requesting assistance to determine client's eligibility for assistance based on program guidelines and restrictions; records clients’ disposition and other pertinent information such as income, expenses, family and work history; assists clients in completing applications for assistance; verifies social security cards and photocopies picture identification cards, payroll statements, utility bills etc.
Assists eligible clients needing transportation assistance based on medical or relocation needs; arranges transportation through bus line and contacts family members, churches for assistance.
Serves as liaison to other agencies in order to obtain/coordinate assistance on client's behalf; builds and maintains professional working relationships with community agencies.
Initiates intake and case plans for Pathway of Hope clients; instructs clients in completing necessary paperwork including goals and/or budget forms; completes orientation check-off form and release of information forms when appropriate; completes social history on new clients each week.
Initiates and prepares case records and logs on all Pathway of Hope assigned clients; ensures the accuracy and completeness of the same; enters pertinent information into the Homeless Management Information System (HMIS).
Meets regularly with clients to discuss and evaluate their progress before transferring client to Pathway of Hope Case Management Specialist; prepares accurate and up-to-date records documenting the same.
Refers clients to appropriate Corps programs or other agencies based on clients’ needs and in accordance with the program plan. Connects clients with Corps Officers in local area.
Makes client referrals and maintain awareness of clients’ progress as directed by Program Director.
Performs data entry of client records; prepares and files client records in a timely manner ensuring all information is accurate and up to date; obtains signatures of clients as needed; reviews and tracks all client records in order to determine if client is active or inactive; maintains confidentiality of client information.
Prepares and maintains statistical records on all services provided; compiles and prepares monthly statistical reports; ensures the accuracy and completeness of the same.
Answers telephone in a courteous and tactful manner; assists clients by providing accurate and complete information regarding the Social Service Program operations and/or services.
Serves as site representative for the Second Harvest Food Bank.
Provides clients with food from pantry; receives and stocks items donated for the pantry; maintains an itemized account of pantry disbursements and inventory. Communicates with Second Harvest the needs and demands of the pantry while ordering items when needed.
Develops current knowledge of services to be administered.
Contacts appropriate service providers to simplify the referral process.
Interviews and screen clients for eligibility.
Maintains client records by category to obtain statistics necessary for programming, development, and agency service agenda.
Monitors interns and volunteers to assist in sorting, stocking, and distribution of food products.
Inputs information into the Food Bank computer network as established by Second Harvest guidelines and procedures.
Pick up food from the Second Harvest Food Bank weekly to fill the food pantry at the Corps
What we are looking for in you:
High School Diploma or G.E.D. preferred but not required,
and
six months experience working with the public with experience work in a security or social service environment preferred,
or
any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.
Physical Requirements and Working Conditions:
Ability to meet attendance requirements.
Ability to read, write, and communicate the English language. Ability to perform mathematical calculations.
Ability to type and keypunch information into a computer.
Ability to sort and file documents alphabetically and numerically.
Ability to operate various general office equipment including a telephone, computer, and calculator.
Ability to drive The Salvation Army vehicle. Work requires the ability to lift and carry and/or push/pull light (less than 25lbs.) materials, supplies, and equipment.
