Promote the services of the agency and evaluate incoming patients and/or referrals to determine their eligibility under Medicare criteria
 

 Essential Duties & Responsibilities

• Promote services of the agency with physicians, hospitals, and long-term care facilities. 
• Obtain physician’s orders for Home Health. 
• Coordinate the pre-admission process with all necessary departments. 
• Provide evaluations of possible clients to determine their current medical status.
• Assist with obtaining necessary residents’ & families’ signatures for the admission’s process. 
• Obtain all required physician’s documentation. 
• Attend all Utilization Review meetings. 
• Coordinate special events to promote the services of the agency. 

Qualifications

• 3-5 years of home health related experience in Admissions/Marketing. 
• RN License.
• Bachelor’s degree preferred 
• Must have a high school degree or equivalent, a must. 
• Must be familiar with Medicare Regulations. 
• Must have excellent communication skills, both verbal and written English. Ability to speak publicly in Spanish preferred. 
• Ability to read and interpret medical documents. 
• Functional math skills to be able to calculate and gather information for statistical use.
