Hospital Clinical Liaison

We’re seeking a full-time Hospital Clinical Liaison to serve as the primary point of contact for collaborative services with hospital partners; acting to enhance continuity of care for shared clients. As a clinical provider, the Hospital Clinical Liaison will provide on-site, mental health assessments to newly referred clients and secures mental health services post discharge as appropriate. Additionally, the Hospital Clinical Liaison will work collaboratively with hospital staff to establish relationships, workflows, and systems for effective communication.
The ideal candidate will have:
- Must be actively licensed by the State of Ohio Counselor, Social Worker and Marriage and Family Therapist Board.
- Prior experience in administering diagnostic assessments and utilizing various mental health-screening tools strongly desired.
- Must have an awareness of and sensitivity to the cultural diversity of the populations served.
- Must have a valid driver’s license in the State of Ohio and access to an insured vehicle
