Position Summary:
Responsible for overseeing the Family Resource Center for San Ysidro Health. Will work cooperatively with the Social Services Supervisor and Social Services Manager and will have a role which is flexible enough to accommodate the diversity of programs and services offered through San Ysidro Health.

Position Location: El Cajon or Jacob's Center with travel to different sites throughout East County.

Essential Functions of the Job:

  • FRC Coordinator to assist and participate in community meetings related to networking for program visibility providing information on community resources and promotion of SYHealth services.
  • Conduct outreach activities to the community to build FRC presence and recognition in underserved areas of the county.
  • Develop collaborative relationships with the community at large and promote the Family Resource Center to families and community providers.
  • Provide internal and external resource information to all scheduled and walk-in patients of SYHealth.
  • FRC Coordinator to collaborate and serve as a liaison with internal departments and deliver presentations regarding Family Resource Center and Social Services.
  • Oversee FRC daily operations, including submitting work orders when needed.
  • Document all patient contacts, complete any necessary reports and records to maintain accurate files for program evaluation and monitoring of caseload.
  • FRC Coordinator to ensure culturally competent processes, service delivery, fidelity of materials and resources for families.
  • Represent SYHC in an effective and professional manner and maintains positive working relationships with the general public, clients, patients, co-workers and other agencies.
  • Adheres to SYHC and all department’s policies and procedure protocols.

Additional Duties and Responsibilities:

  • Maintains established departmental policies and procedures, objectives quality assurance programs, safely, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Attends meeting as required and participates on committees as directed.
  • Performs other related duties as assigned or requested.

Job Requirements:

Education Required:
High school diploma or GED equivalent.

Certifications/Licenses Required:
CA driver’s license and vehicle with appropriate insurance coverage.Experience and Required Skills:
2 years in related Social Services, Public Relations and/or HealthCare Administrations field in a healthcare environment. Experience and knowledge with Microsoft Office software.

Verbal and Written Skills Required to perform the Job:
Good written and verbal communication.

Working Conditions and Physical Requirements:
Prolonged periods of sitting, and constant walking and standing.

Universal Requirements:
Pre-employment requirements include I-9, physical, positive background and reference check results, complete application, new hire orientation, pre-employment PPDs.
