Bilingual Vaccination Community Health Worker/Promotor(a) - On-Site
Position Summary:
Under the direct supervision of the Program Supervisor, the Community Health Worker/Promotor(a) will assist in three main activities (i) provide vaccine education and community engagement, (ii) recruitment of new program participants, and (iii) conduct outreach for the Mobile Vaccination Services (MVS) Program under the Mobile Health Services Department (MHS).
The CHW/promotor(a) will support the MVS program in educating SYHealth patients and community members on the importance the MVS Program merges SYHealth’s immunization services under the Mobile Health Services Department and its COVID-19/Flu Vaccination Initiative. The responsibilities of the CHW/Promotor(a) will include providing education and outreach related to the MVS program; in-person outreach (door-to-door, canvasing, tabling) in the community and at SYHealth mobile clinics locations. Furthermore, the CHW/promotor(a) will be responsible for the registration and referral of potential participant by creation of appointments and referrals; providing general program information; tracking outreach/engagement activities for reporting purposes; and supporting data collection processes. The CHW/Promotor(a) will also be responsible for other duties necessary for the successful implementation and completion of the Mobile Vaccination Services Program as assigned by the Program Supervisor and MHS Manager.
Essential Functions of the Job:
- Conducts culturally appropriate outreach to promote the AoURP among Hispanic, Black/African American, LGBT, rural and other UBR populations
- Attends various outreach and engagement events in both community and clinical settings as assigned by supervisor
- Assist in the setup, operation, and maintenance of research engagement/outreach equipment as specifically instructed
- Recruits and refers potential participants to the AoURP according to study protocols
- Conducts follow-up calls to referred/interested potential participants
- Obtains verbal consent to collect demographic variables from potential participants
- Performs data entry using the study data management system (i.e.; REDCap)
- Assists with routine data verification and quality controls to ensure data integrity and consistency
- Aid in a variety of tasks in support of established recruitment and retention goals. Provide assistances to the Principal Investigator, Program Manager, and Program Supervisor as needed
Additional Duties and Responsibilities:
- Enhances professional growth and development through participation in educational programs
- Attends meetings as required and participates in committees as directed
- Performs other duties assigned
Job Requirements
Experience Required:
- 1-year related experience in public health, health promotion, research, and/or customer service
Education Required:
- High school diploma or GED equivalent
Verbal and Written Skills Required to Perform the Job:
- Bilingual (English/Spanish)
- Excellent oral and written communication skills. Is an active listener and provides support and guidance to others
Technical Knowledge and Skills Required to Perform the Job:
- Superb organizational ability and outstanding social engagement and customer service skills
Equipment Used:
- Computers, Software systems, Scanner, Telephones, Copy and Fax machines. Familiarity with excel and database management software (ex: REDCap)

- Prolonged, extensive, and considerable standing/sitting/walking/lifting

- May be required to work evenings and/or weekends, and provide staffing coverage at any SYHealth facility as needed
