River Edge Behavioral Health is seeking a Core Case Manager. In this role the candidate will Provide a broad range of paraprofessional social services to individuals with mental health, substance use and intellectual/development disabilities and their families. Participates in treatment team planning and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. Performs case management duties.

Duties and Responsibilities: 

  • Provides resource development and linkage to individuals either at home or in a community location of the individual's choice. 

  • Monitors individual's progress towards treatment goals. 

  • Assists clients in locating and utilizing community resources including food resources, medical care, financial assistance, and other referral services. 

  • Provides short-term case management and referral services to clients with emergency situations.  

  • Provides links to housing programs and emergency shelters. Assist with applications for housing programs, income-based housing programs, or other housing placements.  

  • Provides skills training on independent living skills and coping and adaptive skills for managing symptoms of mental illness.  

  • Completes documentation for each service delivery through progress notes.  

  • Completes monthly case reviews.  

  • Monitors and coordinates re-authorizations for treatment, medical appointments, and tracking refills on medications.  

  • Completes assessments for risk of suicide, depression, and substance use. 

  • Works with individuals to promote recovery and independence.  

 

Qualifications 

  • Bachelor's degree in a social service-related field with some experience in a social service-related position 

  • Provide 2 Professional references.  

  • All applicants must pass a satisfactory background clearance and pre-employment drug test.
