River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups. 

River Edge is seeking a dynamic and driven Team Leader to oversee the Care Coordination Services for the Fulton County Permanent Supportive Housing program. This individual will play a vital role in enhancing the effectiveness of the clinical team. This position involves providing guidance, support, and resources to clinicians, ensuring they have the tools necessary to deliver high-quality care to residents. The Team Leader will also facilitate communication between team members, oversee program implementation, and contribute to professional development opportunities.

Schedule:

                        Monday-Friday 8:30am-5:00pm        

Duties and Responsibilities:

  • Provide direct support and supervision to Master Level Clinicians, ensuring adherence to best practices and program standards.
  • Assist clinicians in managing complex cases and problem-solving to enhance service delivery to residents.
  • Foster a collaborative team environment by facilitating regular team meetings, case conferences, and feedback sessions.
  • Ensure effective communication among team members.
  • Identify training needs and coordinate professional development opportunities for clinical staff.
  • Provide mentorship and coaching to enhance the skills and competencies of clinicians.
  • Support the implementation of program policies and procedures, ensuring that clinical services are delivered effectively and efficiently.
  • Collect and analyze data on program outcomes to assess effectiveness and identify areas for improvement.
  • Contribute to the development of reports and presentations for stakeholders and funders.
  • Collaborate with external service providers and community organizations to enhance resources available to clinicians and residents.

Qualifications:

  • Master’s degree in social service (Social Work, Psychology, Sociology, Human Services) required.
  • An associate licensed mental health professional (LMSW, LAPC, LAMFT) required.
  • Demonstrated knowledge of the practices of Harm Reduction and Housing First philosophies required.
  • At least three (3) years of experience working with those with substance use and mental health concerns required.
  • At least one (1) year of experience supervising a high-functioning and autonomous team preferred.
  • At least one (1) year of related experience in housing preferred.
  • Experience working with ClientTrack, the Homeless Management Information System, is a plus.
  • People with lived expertise of incarceration, recovery, homelessness, and those who identify as Trans or LGBQ+ are strongly encouraged to apply.
