River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups. 

Hospital Liaison Transition Coordinator– Baldwin Site

 

Position Summary:

River Edge is looking for an experienced Hospital Liaison Transition Coordinator with a master’s degree or recent graduates in the field of Social Work, Psychology, Counseling, or Behavioral Health related fields.

 

In this role, you will…... Facilitate and coordinate the reintegration of clients from an impatient environment back to the community. Participate in weekly disciplinary treatment team meetings, coordinate community re-entry with referral program, develop community-based treatment plan, participate in court proceedings when necessary.

 

Location/Schedule:

                                    60 Highway 22 West, Milledgeville, GA

                                    Monday-Friday 8:30am-5:00pm

Duties:

  • Duties include building and maintaining relationships with discharge authorities, facilitate discharge by coordinating all appropriate procedures working in partnership with interdisciplinary treatment team, other agencies, ensuring that all plans are in place for the time of discharge to ensure continuity of care.

 

Qualifications:       

  • Master’s degree in social work, Sociology, Criminal Justice, and/or Psychology.
  • Candidates eligible for licensure as LCSW, LMFT, LPC/LMSW, LAMFT, LAPC are preferred.
  • Strong problem-solving and critical thinking abilities to assess patients’ needs, identify potential barriers, and develop effective discharge plan. Exceptional organizational and time management skills to handle cases and deadlines simultaneously.
