Program: Mobile Health - Med Van

Schedule: Monday – Friday 9am - 5pm 


Program Overview:
 

Reporting to the Mobile Operations Manager, the Mobile Health Patient Services Associate is responsible for working in collaboration with clinical staff and collaborative partners to provide administrative and relationship management support to Project Renewal’s Mobile Health Program. Working as a member of Project Renewal’s Primary Care Mobile Unit team--and in partnership with PRIbased programs and external partners-- the Patient Services Associate works directly on the mobile health vehicles to identify and link existing and potential patients to available health services.


Essential Duties & Responsibilities:
 

Identify and link potential patients to available health services, including but not limited to tuberculosis screening, comprehensive exams, preventative care screenings, substance abuse treatment programs and treatment of chronic disease and behavioral health. Tasks include, but are not limited to:

  • Expanding awareness of healthcare services through mobile health program by conducting direct community outreach on location to assigned territories, including shelter residence common areas and surrounding perimeters, faith and community-based organizations such as soup kitchens, transitional and assessment-based housing
  • Serve as liaison to assigned territories, notifying site representatives of upcoming service schedules and cancellations, coordinating lists of patients identified in need of service appointments and other related correspondence
  • Registering clients into the healthcare program using patient registration and appropriate consent forms, and entering contact, demographic and financial data into the Electronic Medical Record (EMR).
  • Scheduling patients for medical appointments for behavioral and primary care services with mobile health units or clinic-based services, based on clinical recommendations and patient preference
  • Support access to health insurance by facilitating communication between patient and enrollment specialists
  • Supporting access to referral-based specialty care, including but not limited to providing referral appointment reminders to patients, conduct follow-up calls for consult notes
  • In collaboration with Clinical Director and Mobile Operations Manager, follow established workflows centered on clinical, safety & security and operations; mitigate challenges and proactively work with Mobile Operations Manager to address barriers that prevent the provision of mobile medical services
  • Keeps office and incentive-based supplies adequately stocked by anticipating inventory needs, placing orders/requests, and monitoring office equipment
  • Conducting on-going research and compilation of health promotion, social-based services and related resources, electronically and in-print, for distribution and use across clinic and mobile health unit settings of care
  • Distributing and collecting post- service patient satisfaction surveys and other related survey instruments as directed.
  • Communicate with the Mobile Operations Manager on current site utilization.
  • Participates in departmental meetings and conferences as representatives of the mobile health programs
  • Conduct other related duties as required

Qualifications:

  • High school diploma or graduation equivalency degree (GED)
  • Minimum one (1) year work experience in a medical office setting
  • Basic competency in Microsoft Office (Word, Excel)
  • Substantial telephone etiquette, verbal and written communication skills
  • Strong time management, multi-tasking, organization, scheduling skills
  • Strong customer service / relationship management skills

 

Preferred Qualifications:

  • Associates Degree
  • Knowledge of clinical procedures usually obtained from a certificate or Associates Degree
  • Knowledge of office procedures usually obtained from a certificate or Associates degree
  • Experience with the eClinical Works (eCW) electronic health record system a plus
  • Experience working with individuals facing mental and/or substance use disorders, and/or homelessness Bilingual / fluency in Spanish a plus
