Oak Hill is seeking a highly motivated and dynamic Licensed Practical Nurse (LPN) to assist in maintaining and improving the health and wellbeing of individuals by monitoring health status and providing appropriate treatments to individuals with Developmental and Intellectual Disabilities to enable their highest quality of life in terms of their physical, emotional, and mental health while adhering to Oak Hill’s policies and procedures.  This person will inspire others and make a life changing impact while achieving professional growth and personal fulfillment.  Oak Hill sets the standard, partnering with people with disabilities, to provide services and solutions promoting independence, education, health, and dignity. Location: Hartford Classroom

Schedule: 35 Hours/Week  Float

About the Position:

The LPN provides a range of services for individuals based on their needs, in their home and in their community. This includes but is not limited to personal care, domestic household services, personal appointments, medical appointments, vocational services, community activities, supervision within the home, and other tasks based upon their individual goals. Applicants with no experience and interested in entering the direct care field with a desire to serve people with disabilities are encouraged to apply to this position.

Before you apply for an internal transfer, it is expected that you will familiarize yourself with the position you are applying for, such as the driving distance, schedule, and the nature of the group home or classroom. It is recommended that you reach out to the program manager to get an understanding of the program BEFORE you commit to applying for a job there. Once a transfer has been granted to you, the general rule is that you are NOT PERMITTED TO RESCIND. This is why it is critical that you familiarize yourself with the position you are applying for. If granted, you will be committing to the new position for a minimum of SIX MONTHS before you are eligible to request another transfer.

For all EDUCATION positions or for GROUP HOMES WITH CHILDREN, all employees requesting a transfer must self-enroll and complete Relias Course "V Endorsement 124" before proceeding.  This does not take very long, and it is a required step in order to be considered.
