POSITION PURPOSE: Safe Connections is a short-term, intensive clinical case management program serving adults (age 18+) with SMI and/or co-occurring disorders who have been repeatedly psychiatrically hospitalized and are not connected to outpatient providers. The goal of the program is to engage and support clients in obtaining stability, improving function, and linking them to ongoing mental health, substance use and community-based services. Safe Connections is comprised of Registered Care Coordinators, Peer Support Specialist, Program Specialist, Program Director, Psychiatrist, and Registered Nurse. The Registered Care Coordinator (RCC) provides clinical care coordination, psychosocial rehabilitation, and recovery services to adult clients, with severe mental illness on an outpatient basis, including adults with co-occurring disorders, and performs related work as required. The RCC conducts face to face and telephonic screenings and services in designated Behavioral Health Units, client homes, offices, and community spaces. In addition, the RCC makes arrangements for and provides direct transportation for client appointments as needed.
REPORTING RELATIONSHIP: This position reports to the Program Director.
ESSENTIAL RESPONSIBILITIES AND DUTIES:
- Provide services for clients eligible for Medi-Cal and/or Medicare clients and enters billed services accordingly into Cerner/SmartCare/other applicable EHR.
- Complete Behavioral Health Assessments to identify clients’ needs for supportive services on an ongoing basis, act as a resource, perform case management services by linking clients to supportive services, resources, and benefits (e.g., housing, vocational, employment, food, psychiatry, primary care providers, substance use treatment and support, social activities, etc.).
- Tailor outreach and engagement strategies to be more effective in achieving linkages to ongoing treatment and services.
- Create detailed documentation of client progress using an electronic health record system.
- Perform various other administrative functions associated with the program including maintaining and updating client hybrid charts, updating tracking logs, creating reports.
- Collaborate with adult clients to develop service plans, specifying goals and objectives, types of services, and therapeutic interventions.
- Request assistance and guidance from management and professional staff to address client issues and problems; establish and maintain contact with community resources to ensure continuity of care (e.g., case managers, conservators).
- Provide client services after regular business hours, when needed.
- Provide direct transportation for clients
- Other duties as assigned by the Program Director.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of bio-psychosocial assessments, group and individual counseling, medication management, crisis prevention and intervention, rehabilitation, community integration, and other support services to maintain clients least restrictive environment.
- Knowledge of California’s state-run and funded health coverage programs and billing requirements
- Knowledge of clinical assessment of clients with severe mental illness, counseling theory, and clinical care coordination principles and best practices.
- Ability to operate office equipment and use of Microsoft Office (e.g., Microsoft Word, Microsoft PowerPoint, Microsoft Excel).
- Ability to plan and organize efficiently and demonstrate positive, professional, and tactful communication skills.
EDUCATION AND WORK EXPERIENCE: The completion of a Master’s degree in Social Work or Counseling from an accredited university with graduate-level internship experience with severely mentally ill adult clients or one to two (1-2) years post masters’ experience working with severely mentally ill adult clients, including those with co-occurring disorder. Trained in and experienced with Motivational Interviewing and CBT interventions preferred.
LICENSES, CERTIFICATIONS & SPECIAL REQUIREMENTS: A valid Class C California Driver’s License; A clear TB test required upon hire and must be maintained every four years. Must be registered Social Worker or Marriage Family Therapist by the California BBS required. Valid CPR Certification.
