Manager, Care Coordination
San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a full time onsite position in our Mission Valley Office. Typical schedule is Monday - Friday, 8:00AM - 4:30PM.
Job Summary: The Care Coordination Manager is responsible for overseeing the effective functioning of the Care coordination department that includes supervisors as well as large coordinator teams. The team processes PCP referrals to contracted specialist offices and schedule medical and surgical appointments on behalf of PACE participants. The manager provides leadership, guidance, builds and maintains effective and efficient processes, performs basic team management and ensures the department meets its goals and objectives. This individual will also be responsible for evaluating department performance and taking corrective action as needed.
Duties and Responsibilities
- Ensure the department maintains compliance with PACE regulations regarding timeliness of care all the time.
- Ensure PACE participants are receiving the care in a timely fashion and troubleshoots issues on a daily basis.
- Ensure the teams follow standard processes and conduct process improvements as needed.
- In collaboration with supervisors, keep a pulse of the day-to-day activities of the teams, identify operational and procedural issues, gaps or shortfalls and resolves them effectively and timely.
- Manages personnel including recruitment, onboarding, ongoing training, feedback and performance monitoring. Monitor operations and performance through audits and measure against carefully crafted key performance indicators (KPI) and respond as needed to maintain the KPIs within the desired parameters.
- In collaboration with the contracts department, initiate new contracts, maintain communications and troubleshoot provision of care as well as billing issues that negatively impact participant care.
- Assign tasks, delegate responsibilities and ensures staff accountability for expected functions.
- Work closely with the IDT and the Transportation Department to ensure collaboration and efficiency
- Act as a liaison between various departments and communicates with PACE centers and participant IDTs to ensure consistency and collaboration
- Follows up on participant grievances and spearheads satisfactory resolutions in collaboration with PACE Centers and Quality and Compliance departments.
- Ensure compliance with company, State and Federal policies and procedures.
- Woks with Vice President of PACE services to implement changes
Requirements and Qualifications
- Required 5 years of management experience in healthcare or related field MPH, MHA or similar education preferred.
- Required above basic experience with Electronic Medical Records (EMR) with administrative responsibilities
- Required experience with medical terminology.
- Experience managing large, multi-tiered teams is highly preferred
- Experience working with medical offices, providers and contracts is preferred
- Knowledge and experience with LEAN or Six Sigma for process improvements and standardization of processes and systems highly preferred.
- Experience with reporting and data presentation as well as using data management or project management tools like PowerBi, Excel (advanced), Smartsheets, Visio or others to build dashboards or project plans and is preferable.
- Excellent communication and interpersonal skills
- Strong analytical and problem-solving skills
- Able to multitask and prioritize tasks efficiently
- Proficient in Microsoft Office Suite and other related software programs
