Field Care Coordinator Supervisor

JOB SUMMARY:
Responsible for the day-to-day operation and performance of Care Coordinator Team as the members of Interdisciplinary Team within the compliance with CPHL Care Management policies, procedures and all regulatory mandates, consistent with CPHL mission, vision and value statements.
The Field Care Coordinator Supervisor is responsible for continuously improving the effectiveness and efficient operations of the Care Management Department. Functions as a resource for the management staff to maximize quality of standards of care and appropriate utilize of resources. Works with other departments and the plan providers to maximize cooperative relationships and acts as an interface with others for excellent customer service and process improvement.
PRIMARY RESPONSIBILITIES:
- Create and maintain all training manual/documents used to onboard Care Coordinators,
- Onboard and educate all new Coordinators in scheduling and completing visits, data entry, and other support tasks.
- Directly oversee the Field Coordinators and ensure compliance with productivity and documentation requirements.
- Provide Coordinators with feedback on quality of work while guiding them towards improvement.
- Identify areas for and assist in education and training as needed to meet all CPHL standards, ensuring that Field Coordinators operate within all CPHL policies and procedures.
- Continuous supervision of field staff to ensure company protocols are in place.
- Maintain departmental schedules, submits regular reports on a weekly basis.
- Assist and completes staff evaluations.
- Complete Bi-weekly pay roll and travel reimbursement reports
- Additional assignments and duties as needed.
Education and Experience:
Education
Required:
- Minimum 2 years professional experience in related field
Preferred:
- College degree
- Professional supervisory experience
Required:
3-5 years previous Home Care and/or Managed care experience which include administrative responsibilities
Knowledge and Skills
Required:
- Excellent written and verbal communication skills
- Proficiency in the use of Microsoft Office tools
- Administration and organizational skills
- Ability to perform independently and prioritize projects and assignments
- Interpersonal skills
- Strong multi-tasking abilities
- Exercises discretion in relationships with internal and external contacts, understands and adheres to confidentiality and proprietary requirements.
- Understanding of and sensitivity to cultural differences and needs of the community are essential. Must be adept at operating within a diverse and multi-cultural work environment and community of members
Preferred:
- Knowledge of MLTC regulatory requirements
