UAS Coordinator

JOB SUMMARY:
The Care Coordinator works within a multidisciplinary care team to help assist with the care of members enrolled in CPHL Managed Long Term Care program and other LOBs to facilitate, schedule and arrange a variety of treatments and healthcare related activities.
REPORTING RELATIONSHIPS:
Reports to Care Coordination Manager and shares a collaborative relationship with the Care Management Team and other departments involved in member’s care.
PRIMARY RESPONSIBILITIES:
- Assists in scheduling of ongoing member visits such as UAS Assessments for the Field Assessors which includes tracking and rescheduling of canceled appointments.
- Must reach daily scheduling quotas
- Provide a daily report of all scheduled visits
- Assists with uploading all UAS documents into members files accordingly
- Generates member correspondence and handles mailings as needed.
- Coordinates communication between Field Assessors and other members of the multidisciplinary team
- Documents daily calls and follows care coordination documentation guidelines which includes but are not limited to: documentation of problems, interventions and outcomes.
- Acts as liaison and member advocate between the member/family, physician(s) and the care team to ensure timely and effective communication in regards to scheduling the member’s assessment
- Participates in on-going education activities
- Ensures that all job responsibilities are carried out in compliance with CPHL policies, NY State and Federal regulations.

Required: HS Diploma
Preferred: Bachelor’s Degree

- 2 years customer services experience
- Strong written and verbal communication skills
- Computer proficiency
- Strong multi-tasking abilities
Preferred:
- Experience working within a managed care environment
- Experience working with the geriatric population
- Bilingual
Specific Technical Skills:
Preferred: Computer proficiency in Microsoft Office applications, Excel
