Community Intervention Specialist
Department of Health and Human Services
Hourly Rate: $27.66 - $32.20

Job Summary: Under direction of the Deputy Administrator, the Community Intervention Specialist is the critical link between homeless outreach and permanent housing placements in the community. Services include benefit establishment, linkage to mental health and substance abuse treatment, negotiating with landlords, and assisting consumers in finding and maintaining housing.
Job Duties:
- Assist with housing placements for homeless individuals. Collaborate with homeless outreach workers and various case management organizations to ensure successful housing placement.
- Coordinate of care. Maintains linkage with outreach teams to find successful housing placements.
- Work with existing and potential landlords to find additional units and to ensure a quality of housing for homeless placement.
- Assist program participants with benefits acquisition to include gaining income, insurance, and other benefits.
- Participate in the Milwaukee Continuum of Care by attending monthly meetings and participating in sub-committees.
- Up to 70% travel within the community.
- Perform other duties as assigned.
Minimum Requirements:
- Current resident of the State of Wisconsin.
- Current Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment.
- Must be able to travel extensively within the community.
- A Bachelor’s degree or higher in Social Work or a Human Services related field from an approved college or university.
- At least one (1) year of experience working with the homelessness population, in the housing field, human services, or closely related field.
- At least and Intermediate know of all computer and software applications such as Microsoft Word and Microsoft Excel.
Knowledge, Skills, and Abilities: Knowledge of housing programs and HUD regulations as they relate to homelessness and income guidelines; knowledge of available housing resources; knowledge of the permanent supportive housing model; ability to conduct clinical assessments to provide for proper discharge planning; ability to work with individuals with a wide range of needs.
