A New Leaf is seeking a dedicated Case Manager to provide support and advocacy for individuals and families experiencing housing instability. This role focuses on assessing client needs, developing individualized service plans, and connecting clients to essential community resources to promote long-term housing stability and self-sufficiency.

 

Hourly Pay Range: $22 - $24 DOE

Schedule: Monday – Friday; 8 AM – 5 PM (in-office)

Location: Surprise, AZ

 

QUALIFICATIONS

Education & Experience:           

  • High School Diploma/GED required AND
  • 4+ years of direct Case Manager Experience required
  • Experience working with individuals experiencing homelessness or those who are economically disadvantaged preferred
  • Knowledge of community resources

OR

  • Associates or Bachelors Degree AND 
  • 3-4 years of direct Case Manager Experience required
  • Experience working with individuals experiencing homelessness or those who are economically disadvantaged preferred
  • Knowledge of community resources

DUTIES AND RESPONSIBLITIES. Responsibilities will include but not limited to: 

  • Provide case management services; establish/maintain client files.
  • Being directly responsible to Program Manager for issues regarding admission and discharge, while being in conformance with the agency’s program definition and policies as relevant to program/client needs.
  • Screening, assessing and developing service plans.   
  • Developing/Monitoring the implementation of strength-based case plans and progress towards goals.
  • Assisting clients in obtaining community services and benefits, securing appointments with outside services and agencies as needed.
  • Providing referrals and/or coordinating transportation for clients to community services as needed (as relevant to program/client needs).
  • Facilitating and coordinating discharge planning transition and follow up according to plan.
  • Completing accurate case documentation and agency paperwork in a timely, correct, and thorough manner.
  • Maintaining client demographic information and accurate tracking as required by supervisor.
  • Providing educational information as needed to assist clients/families in reaching goals.
  • Attending and participating in program staff meetings and/or any other meetings as designated by your supervisor(s).
  • Other tasks deemed necessary per program supervisor do to changes in program design.

Compliance/Requirements:

  • Class One Fingerprint Clearance Card 
  • Valid Driver’s License with reliable transportation 
  • Current Auto Liability Insurance 
  • 39 Month Motor Vehicle Record 
  • Must Be At Least 21 Years of Age
