A New Leaf is seeking a compassionate and dedicated Family Connections Consultant to join our Family Connections Division. In this vital role, you will provide essential support to families, guiding them through challenges, strengthening family dynamics, and developing customized strategies that empower families to build healthier, more resilient relationships. As a Family Connections Consultant, you’ll have the opportunity to create lasting change and make a profound impact on the families you serve.
As a Family Connections Consultant, you will be at the forefront of providing critical case management and connecting families with essential resources. Your expertise will help families strengthen their protective factors, reduce the risk of child abuse or neglect, and ensure children can remain safely in their homes. Hourly Pay Rate: $22 per hour
Location: Maricopa County, AZ

Whether you prefer daytime hours or are open to evenings and weekends, we will work with you to design a schedule that fits your lifestyle while accommodating the families’ needs.
Required Orientation: New hires must complete our "New Hire Orientation," held Monday-Friday, 8:30 AM to 5:00 PM at our HR Campus in Mesa, AZ. Early release may be granted based on completion of orientation requirements. Orientation is not available evenings or weekends.
QUALIFICATIONS
Education: Bachelor’s degree in Social Work or a related field.
Experience: At least 1 year of experience in the human services field.
Skills: Demonstrated understanding of family dynamics and family-centered services.
Knowledge: Expertise in community resources, cultural diversity, substance abuse, child development, and trauma-informed care.
Physical Requirements: Ability to transport clients safely, drive personal vehicle for business-related travel, and maintain accurate mileage records.
Key Responsibilities:
- Provide direct case management services to families whose children are involved with the Department of Child Safety.
- Conduct in-home visits, assessments, and intervention services to enhance family dynamics and ensure safety.
- Collaborate with families to develop tailored service plans addressing mental health, parenting, substance abuse, domestic violence, and other related challenges.
- Assist families in accessing community resources, such as social services, financial support, healthcare, education, and employment services.
- Monitor, observe, and document all interactions between parents and children during home visits, providing support when necessary to de-escalate situations.
- Work closely with the Department of Child Safety to develop assessments, provide regular reports, and attend necessary meetings.
- Provide education on substance abuse and assist families in linking with treatment providers when needed.
- Ensure all documentation, including progress notes, activity reports, and incident reports, is submitted accurately and on time.
- Establish and maintain confidential service records for each family and provide ongoing support throughout the service delivery process.
