Hospice Team Coordinator
JSSA is looking for a full-time Team Coordinator to join our wonderful Hospice administrative team in providing overall administrative and scheduling support to the clinical team.
Responsibilities:
- Schedules hospice aide staff and coordinates cross coverage of hospice aides during absences
- Administers Durable Medical Equipment (DME)
- Helps with managing the supply room inventory
- Prepares EMR Workflow and Reports and assists with IDG meetings
- Assists with all documents' verifications, transfers, revocations, releases and ensures they are completed, signed and processed within established time frames
- Scans/uploads forms into the EMR document library
- Reviews and responds to telephone messages, emails and in-person patient requests for information
- Answering phones as a member of the call center and helps provide front desk coverage as needed
- Participates in all required meetings
- Promptly reports all incidents and/or issues to Nurse Manager or Office Manager as appropriate
Qualifications:
- High School Diploma or equivalent, associate degree preferred
- 3-5 years’ experience performing related work
- Demonstrated organizational and time management skills
- The ability to multitask and work in a fast-paced environment
- Demonstrated flexibility, adaptability, and outstanding customer service skills
- Proficient with MS Office 365,Electronic Medical Records and other databases
- Familiarity and comfort with medical terminology
- Ability to manage confidential information with sensitivity
- Capable of communicating professionally with patients, power of attorneys, legal guardians, primary care physicians and other community/medical providers
- Outstanding phone and in person customer service skills
- Able to work in the office Monday through Friday from 8:30am-5pm
