How you can make a bigger difference

The RN is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice.  The RN is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities.  The RN must possess a working knowledge of NYS rules and regulations and JBFCS policies and procedures.  The RN must be capable of supervising, understanding and working effectively with diverse populations.

 

KEY ESSENTIAL FUNCTIONS:

 

  • Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations.
  • Uses nursing process to make continuing assessments of our client’s total health needs.
  • Perform nursing assessments, by direct physical examination.
  • Develop and implement an individualized plan for nursing service.  The nursing care plan must be updated at least annually or whenever there is significant change in the client’s condition.
  • Coordinate all acute and ongoing health services, as indicated by need and regulatory mandate.
  • Monitor health and behavioral status, labs and efficacy of prescribed treatments.  Ensure timely communication of findings to appropriate clinical personnel.
  • Ensure that all medical record documentation is accurate, timely and outcome based and reflected in the nursing notes.
  • Provide health education to all clients and their support system.
  • Provide initial and ongoing, but at a minimum frequency of annually, training to staff  in all nursing tasks, functions, protective and preventive health measures.
  • Provide training and ongoing support to staff regarding medication administration, infection control, and other training deimed appropriate.
  • In conjunction with the Director of Nursing, interview and determine candidacy of other RN and LPN applicants.
  • Responsible for supervision, discipline and scheduling of L.P.N’s staff hours.
  • Maintain documentation of all training curriculums, attendance and post training testing.
  • Must have experience and ability in working with electronic health record management, electronic care coordination and electronic health record interface with external service providers.   
  • Active participant in the Interdisciplinary Team (IDT).
  • On call availability.
  • All other duties, as assigned.

 

EDUCATIONAL / TRAINING REQUIRED:

 

  • Active, unrestricted New York State license as a Registered Professional Nurse.

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE (List what the desire to reach goals):

 

•             Managerial experience preferred.

•             Must possess excellent verbal and written communication skills.

•             Ability to work in a team environment.

•             Must be able to work independently and possess sound independent judgment and decision making skills.

 

 COMPUTER SKILLS REQUIRED:

 

  • Proficiency in Microsoft Office Suite applications required.

VISUAL AND MANUAL DEXTERITY:

  • The candidate should be able to read documents for analytical purposes such as health charts, spreadsheets, etc.
  • Possess the manual dexterity necessary for handling medical equipment, administering medications, and performing clinical procedures.

 

WORK ENVIRONMENT/PHYSICAL EFFORT

  • Ability to walk, sit, stand, and bend as necessary to assess and provide care to clients effectively in various settings.
  • Must be capable of responding swiftly to emergencies, which may involve moving quickly to the scene, performing CPR, and utilizing appropriate emergency protocols.
  • Proficient in lifting, transferring, and repositioning clients safely when needed, ensuring the comfort and safety of both clients and staff.
  • Maintain the physical stamina to perform nursing duties throughout the shift, including prolonged periods of standing and walking, while remaining attentive to patient needs.
  • Ability to navigate a variety of environments, including tight spaces, while maintaining a focus on patient care and safety.
