Purpose:
Home Based Crisis Intervention (HBCI) is a short term, intensive service to avert unnecessary psychiatric hospitalizations of children and youth ages 5 to 20 years. This goal is accomplished via clinical interventions with the child/youth and their family, and case management services such as referrals to longer term services.

Position Overview: 
The Crisis Interventionist is responsible for providing treatment to the child and their family/caregivers to address the clinical needs of the child and the complex needs of the family unit. The Interventionist will provide crisis intervention and stabilization with the child/youth and family, and case management services such as referrals to other services and ensuring the structural home environment is safe. Treatment interventions are to be individualized to the child/family and evidence-based practices should be utilized to address identified clinical and family system needs. This position requires flexible evening hours and rotating overnight phone coverage.

RESPONSIBILITIES include but not limited to:

  • Completes intake with all new clients, including risk assessments and psychosocial assessment, within first 3 contacts with the child/family.
  • Works with client and family to create safety plans.
  • Develops and implements short-term service plans for clients in coordination with client/family, team members, and other current service providers.
  • Conducts 2 to 3 weekly visits with clients in the community. During visits utilizes a variety of evidenced based practices to provide short-term clinical Interventions designed to reduce risk via the identification of effective coping skills and encouraging resistant clients to accept appropriate behavioral health interventions.
  • Identifies appropriate internal and/or external treatment services and facilitates client referral to identified programs.
  • Maintains all case documentation in the agency’s electronic health record system in accordance with program documentation submission guidelines.
  • Participates in interdisciplinary team meetings and case conferences.
  • Participates in audit preparation.
  • Performs community outreach and establishes linkages with other groups, individuals or organizations which may be helpful to our clients.

CORE COMPETANCIES:

  • Clinical Training and Knowledge of mental illness, serious emotional disturbance and substance use disorders.
  • Ability to document clinical assessments, plans and interventions

Educational / Training Required:

  • Licensed mental health providers (including Licensed Clinical/Masters Social Workers, Licensed Psychologists, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, and Licensed Creative Arts Therapist) preferred. However, will consider candidates with a Master’s degree with at least one year experience in a related area (Social Work, Psychology, or a related health/human services field) or Bachelor’s Degree with at least 4 years of experience in Community based mental health or Case Management.
  • Proficient reading and writing English. Pay differential for bi-lingual clinicians.
  • Valid NYS Driver’s License Required.

 

Experience Required / Language Preference:

  • Minimum of one year of relevant experience with similar client population. Proficient reading and writing English.  Pay differential for bi-lingual clinicians if relevant to the population serviced at site


Computer Skills Required:

  • Working knowledge of Outlook, Word and Excel

Work Environment / Physical Effort:

  • To perform the essential functions of this job the candidate must be able to travel within New York City carrying equipment such as a notebook, forms, laptop, mobile hotspot and cell phone weighing up to approximately 10 pounds.
  • Risks/hazards associated with the position are those which may be encountered travelling around New York City and interfacing with clients experiencing an acute mental health crisis or serious emotional disturbance.
  • Position is currently hybrid but can become 100% in person based on program need.
