General Summary: Provides direct support and supervision for vulnerable adults (members) in home settings and in the community, utilizing therapeutic approaches compatible with Intermountain Centers for Human Development treatment philosophy, with an emphasis on empirical behavior techniques.

JOB RESPONSIBILITIES:

  • Assists in the development and implementation of activity plans for members who are participating in agency programs.
  • Works with families and members to achieve the goals described in the individual service plan.
  • Ensures continuity of member’s program through program data sheets, community outing sheets, and other documentation.
  • Maintains a daily log of pertinent events and information to be communicated with other appropriate staff and professionals; completes other documentation as required or requested by supervisor in charge of setting.
  • Maintains member files in accordance with ICHD and contractual standards.
  • Reports variables that might affect the member (e.g., illness, relocation, medication).
  • Monitors self-administration of medication.
  • Complies with applicable legal, regulatory and workplace reporting requirements (e.g., mandatory abuse and neglect reporting).
  • Communicates effectively with members, families, supervisor and other stakeholders.
  • Briefs families and caregivers regarding member needs and progress (e.g., providing verbal summary of sessions, confirming pick up/drop-off times, requesting hygiene supplies).
  • Maintains daily physical condition of agency vehicles, to include daily mileage logs and maintenance logs to be submitted to supervisor per time scale given.
  • Follows agency purchasing and accounting procedures.
  • Completes all training required by licensing and/or agency, or as requested by supervisor.
  • Maintains professional boundaries with members, families and other stakeholders.
  • Maintains an approved schedule, acceptable level of attendance, and accurate timesheet
  • Works as a member of a team serving members and families.
  • Perform other duties as assigned or necessary as they relate to the general nature of the position.

QUALIFICATIONS:

  • Education – High School diploma or GED required.
  • Experience – One year verified experience volunteering or working with individuals with autism spectrum disorder (ASD) or related disabilities.

 

REGULATORY:

  • Minimum 21 years of age
  • Arizona Driver’s License, 39-month Motor Vehicle Report and proof of vehicle registration and liability coverage to meet insurance requirements.
  • Must possess Level I fingerprint clearance.
  • CPR and First Aid certification.
