SUMMARY: The Extended Foster Care Success Coach engages youth in a coach-like way, and provides strengths-based case management and interventions as needed for youth and young adults preparing for and participating in the Extended Foster Care Program. EFC Success Coaches carry an active caseload of up to twenty young adults.
RESPONSIBILITIES:
- Participate and/or facilitate team-based planning meetings at the invitation of the youth such as wraparound meetings, family engagement meetings, and permanency or community roundtables.
- Build a supportive relationship with youth/young adult that assists them to develop goals and a plan for achieving those goals. Monitor and adjust the plan with the youth based on progress toward goals until the completes Extended Foster Care.
- Assist youth/young adults secure necessary services and supports identified in the plan through warm hand-offs and other active methods. Methods which include obtainment of permanent connections to caring adults, cognitive and social-emotional competencies (life skills), opportunities for educational programming, employment, housing and person-centered transition planning . Address barriers to services through the Local Inter-Agency Team.
- Supporting youth and young adults in crisis stabilization and identifying and addressing the root cause of the crisis. Providing interventions to address immediate safety until the young adult can start or continue in goal progression.
- Act as a liaison with community service providers. Coordinate with service providers to ensure that the youth/young adult’s needs are being met. Develop linkages and collaborate with community service providers and other key stakeholders.
- Perform other duties as required.
QUALIFICATIONS:
Education:
- Shall possess a Bachelor or Associate's Degree in Social Work, Psychology, Counseling, Marriage and Family Therapy, Behavioral Health, or Education. (or equivalent college credits to equal an Associate's Degree) or higher.
- In lieu of degree or credits, an Extended Foster Care Success Coach may posses skills that the coach acquired through alternative routes such as relevant job training, community college attendance, military service or an apprenticeship.
Experience:
- Experience working directly with youth/young adults ages 14-26 who are involved in systems of care, which may include foster care, juvenile justice, runaway and homeless youth progreams.
- Staff with lived experience in systems of care, which may include foster care, juvenile justice, run-away and homeless youth programs is preferred when hiring.
- Exhibit the belief that all young people have the capacity to be successful in their lives.
- Flexible, solutions-oriented attitude and be comfortable trying new approaches.
- Knowledge of trauma-informed care and healing-centered approaches.
- Understand the community resources and providers to ensure successful service referrals
- Good knowledge of human services principles and practices.
- Relevant experience in case management.
- Qualifications set by the lead agency
REGULATORY:
- Minimum 26 years of age.
- DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment).
- CPR, First Aid, AED certification, if required (must possess upon hire and maintain throughout employment).
- Current, valid Arizona Driver’s License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements.
- Initial current negative TB test result, if required (Employer provides).
