GENERAL SUMMARY: The Community Counselor provides strength-based interventions to build self- regulation skills and develops positive coping strategies for anger, impulse control, depression, anxiety and stress, conflict resolution and general self management. Services will be provided to member and/or family in a community, educational, or home-based setting. Care coordination and Child and Family Team (CFT) facilitation skills will also be required and provided as needed based on the needs of the member. Community counselor may also assist in the facilitation of group counseling sessions on a rotating basis.
JOB RESPONSIBILITIES:
- Provides direct family centered clinical services to members as well as care coordination to meet the needs of the member and employee productivity standards. Services include individual, family, and group skills building interventions and counseling services to address a variety of complex issues such as behavioral challenges, parenting skills, family systems issues, anger management, and substance abuse interventions.
- Prepares written diagnostic assessments, assessing service recipients from multiple perspectives, including family functioning, psychological/emotional, social, academic, medical, behavioral, substance abuse, skills, strengths, barriers to self-sufficiency, treatment history, and others, and utilizing various assessment instruments.
- Facilitates Child and Family Team (CFT) meetings. Collaborates and coordinates with other service providers, system partners, and professionals, such as attorneys, school personnel, social workers, and probation officers.
- Interpret and follow treatment plan from other providers when clinically appropriate.
- Responds to crisis situations during the workday as needed.
- Documents case activities for both clinical and billing purposes in accordance with agency and AHCCCS standards.
- Works with Clinical Supervisor to increase effectiveness of therapeutic intervention.
- Performs other duties as assigned or necessary as they relate to the general nature of the position.
QUALIFICATIONS:
- Education – Associates degree preferred, high school diploma or GED required.
- Experience – Minimum 4 years of direct member care experience in Behavioral Health.
- Special Job Requirement – Own, reliable transportation; mileage reimbursement provided.
REGULATORY:
- Minimum 18 years of age.
- Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
- CPR, First Aid & AED certification, if required (must possess upon hire and maintain throughout employment).
- Valid Driver’s License, 39-month Motor Vehicle Report, proof of vehicle registration and liability insurance that meet company insurance requirements, if required.
- Negative TB test result, if required (Employer provides).
