JOB SUMMARY:

Sycamores provides a spectrum of equitable mental health services through various programs to multicultural populations of all acuities. Although it serves a variety of consumers, the agency focuses primarily on providing care to low-income, high at- risk youth.

 

Care Coordination is a targeted case management service in IFCCS that facilitates assessment of, care planning for and coordination of services. Ensures that medically necessary services are accessed, coordinated and delivered in a strength based, individualized, family/youth driven and culturally and linguistically relevant manner and that services and supports are guided by the needs of the child/youth. The role of the Care Coordinator is to coordinate and organize formal and informal supports to partner with families to develop and implement a plan that is permanent in the community. The Care Coordinator does this through an organized facilitated planning process.

STARTING PAY RANGE:

$71,000 - $76,325 per year for Full-Time employment (starting pay will be based on previous work experience and educational background.)


JOB QUALIFICATIONS:

  • Masters or Doctoral degree from an accredited or state approved program that meets criteria for licensing as a clinical social worker, marriage and family therapist or clinical psychologist in the State of California.
  • Licensed eligible or Licensed preferred in the State of California
  • Bachelor’s degree may be considered depending on experience
  • Two or more years of direct practice experience working with children experiencing emotional/behavioral challenges
  • Experience with targeted case management
  • Maintains all required licenses and certifications
  • Bilingual in Spanish Fluency required
