890 Hayes Street (890) is residential Substance Use Disorder (SUD) treatment program that serves approximately 90 male/male-identifying clients. Within 890 is also Acceptance Place (AP), a 10 bed program focused on the SUD treatment of gay men. All 890 and AP clients are supported by an interdisciplinary team of SUD and behavioral health professionals who provide counseling, education, and skills training, in addition to individual and group therapy, for 90 days (or longer based on medical necessity). Our goal is that our clients complete their program with the resources and skills necessary for recovery and are prepared to work independently on the Health & Wellness goals they define for themselves. The Licensed Vocational Nurse (LVN) is an integral part of this interdisciplinary team and supports those in early recovery as they learn to navigate their treatment plans and the healthcare system.
Job Summary
The LVN utilizes the Nursing Process, Harm Reduction, and Trauma Responsive Care modalities in all actions taken to advocate for and support the client in their planning and goals. They focus on supporting clients in developing strategies for taking medications effectively by educating them (within their scope) on how where medication management supports recovery. Acting as a resource and shift lead for unlicensed Health & Wellness staff, the LVN ensures client adherence to evidence-based treatment plans. They triage acuity of other medical needs, report changes in condition to the Registered Nurse and/or Medical Director, and/or implement emergency interventions, as needed. The LVN mentors clients in autonomous goal setting and in building the skills needed to reach those goals. The LVN will perform Tb skin testing when necessary, and other duties as assigned.
Responsibilities
- Monitors patient medication use and helps to mitigate risk to patient by flagging high dose medications and potential medication interactions to the appropriate prescribing providers
- Monitors client response to treatments and initiates nursing or medical interventions, as needed
- Supports Medication Assisted Treatment adherence through counseling and education
- Collaborates with SUD counselors and/or other internal and external stakeholders to dissolve treatment obstacles
- Utilizes the electronic health record as the primary source for all client care appropriately and documents relevant nursing interventions on time
- Works collaboratively with all staff across the HealthRIGHT360 care continuum
- Functions as a nurse within the scope of California LVN licensure, and credentialing and privileging of the organization
Position Requirements
- Active California LVN license
- BLS & First Aid Certification, or must obtain within 30 days of hire
- Able to work with a diverse population with Cultural Humility
- Able to prioritize a Harm Reduction approach
- Interest and/or experience in working with unhoused populations, Substance Use Disorder and Behavioral Health treatment settings, those with criminal backgrounds, and/or people with other potential barriers to economic self sufficiency
