HealthRIGHT360’s Withdrawal Management Unit (WM) is co-located within 815 Buena Vista Avenue West (815) and provides 52 beds to adult men and women for social model WM support, often in the setting of co-occurring behavioral health and Substance Use Disorders (SUD). The WM LVN provides clients with Medication Assisted Treatment (MAT) and supportive care as part of an interdisciplinary team that includes, but is not limited to, SUD counselors and behavioral health professionals. The average length of WM treatment is 7 days but may be extended based on acuity, after which clients that meet medical necessity are eligible to transition into the residential treatment phase of our continuum of care.
Job Summary
The WM LVN utilizes the Nursing Process and Harm Reduction and Trauma Responsive Care modalities in all actions taken to advocate for and support the client in their planning and goals. They specifically focus on co-managing (with clients) any self-administered inductions and/or titration of MAT (prescription medications) and the WM LVN monitors concurrent vital signs and symptoms with CIWA/COWS scoring. They triage acuity, report changes in condition to the Registered Nurse and/or Medical Director, and/or activate emergency interventions, as needed. The WM LVN acts as a shift lead and resource for unlicensed Health & Wellness staff and ensures adherence to evidence-based treatment plans. Where clients need extended WM support they work with the WM Medical Case Manager to initiate authorization. They screen those clients who plan to enter residential treatment for Tb and perform skin testing as needed, and other duties as assigned.
Responsibilities
- Monitors patient medication use and helps to mitigate risk to patient by flagging high dose medications and potential medication interactions to the appropriate prescribing providers
- Monitors client response to treatment and initiates nursing or medical interventions, as needed
- Supports Medication Assisted Treatment adherence through counseling and education
- Collaborates with SUD counselors and/or internal and external case managers to dissolve treatment obstacles
- Utilizes the electronic health record as the primary source for all client care appropriately and documents relevant nursing interventions on time
- Works collaboratively with all staff across the HealthRIGHT360 care continuum
- Functions as a nurse within the scope of California LVN licensure, and credentialing and privileging of the organization
Position Requirements
- Active California LVN license
- BLS & First Aid Certification, or must obtain within 30 days of hire
- Able to work with a diverse population with Cultural Humility
- Able to prioritize a Harm Reduction approach
- Interest and/or experience in working with unhoused populations, Substance Use Disorder and Behavioral Health treatment settings, those with criminal backgrounds, and/or people with other potential barriers to economic self sufficiency
