The Transitional Housing (TH) Programs provides transitional housing to men and women for up to nine months after they have completed the residential Substance Use Disorder (SUD) treatment phase of our care continuum. Clients are supported by a team of Recovery Coaches and Outpatient Services staff such as SUD counselors and behavioral health clinicians, and they receive counseling and education for SUD, life skills, as well as individual and group therapy. Our goal is that our clients graduate with the skills and resources to sustain a steady income, affordable housing, and optimal well-being, and the Licensed Vocational Nurse (LVN) is an integral part of this interdisciplinary team that supports our client’s recovery while they learn to navigate the healthcare system independently.

Job Summary
The TH LVN utilizes the Nursing Process, Harm Reduction, and Trauma Responsive Care modalities in all actions taken to advocate for and support the client in their planning and goals. They will specifically focus on supporting clients as they develop their own strategies for taking their medications effectively, including but not limited to MAT (Medication Assisted Treatment) prescriptions such as Buprenorphine, Buprenorphine-Naloxone, and/or Methadone, by educating clients (within their scope) on how medication management supports Recovery. The TH LVN will closely collaborate with the Addiction Medicine and Medical Directors and the Behavioral Health Director of Nursing if/when clients experience recurrences of substance use so that timely and safe interventions are implemented. They will reconnect existing clients with the MAT team for routine medication management, as needed, and other duties as assigned. 

Responsibilities

  • Functions as a nurse within the scope of California LVN licensure, and credentialing and privileging of the organization
  • Works collaboratively with all staff across the HelathRIGHT360 care continuum
  • Supports Medication Assisted Treatment through counseling and education
  • Monitors patient medication use and helps to mitigate risk to patient by flagging high dose medications and potential medication interactions to the appropriate prescribing providers
  • Supports care coordination for clients accessing healthcare services internal and external to HealthRIGHT360
  • Utilizes the electronic health record as the primary source for all client care appropriately and documents relevant nursing interventions on time

Position Requirements

  • Active California LVN license
  • BLS & First Aid Certification, or must obtain within 30 days of hire
  • Able to work with a diverse population with Cultural Humility
  • Able to prioritize a Harm Reduction approach
  • Interest and/or experience in working with unhoused populations, Substance Use Disorder and Behavioral Health treatment settings, those with criminal backgrounds, and/or people with other potential barriers to economic self sufficiency
